 Equip Transitions Coordinator – Salary £22,180 Purpose The role of Transitions Coordinator is to ensure the smooth transition of people with learning disabilities and/or Autism into the Equip project. Working directly with young people and their families and supporting the Senior Equip Team, you will form a crucial part of the next steps for young people as they move out of school and into college or community education. Overview The Transitions Coordinator will meet with young people and their families to coordinate a plan that ensures the Equip project has everything in place for their education. This will include logging EHCP plans, producing student files, undertaking an initial assessments writing and updating risk assessments, filing all completed documentation and contacting external agencies including CAMHS to identify potential joint working pathways. This role is key to the young person’s transition and will have ongoing responsibilities as the young person continues through their education pathway. Reporting to the Quality and Communications Specialist, this coordinator will be required to attend open evenings to promote the work of the Equip project and to attend and support student annual reviews. Culture and Practice As the first point of contact for young people and their families joining the Equip project, the Transitions Coordinator role is vital in demonstrating the Talkback approach. Having a real passion for ensuring that people have clear information and knowledge about how their future and how they will be supported is crucial to this role. All team members need to be confident in communicating a message and listening to the needs and concerns of the people we support, their families and other agencies. Modelling best practice as a member of a large and developing team, as well as with external agencies is a key part of this, and any role within Nclude. Person Specification Experience, skills and qualifications  Minimum of at least one year’s experience in an administrative role  Excellent communication and interpersonal skills that include report writing and verbal skills  Well‐developed IT skills including Word, Excel and database systems.  Ability to work on own initiative and be able to initiate and adopt new ideas quickly by having a flexible approach.  Experience of handling confidential information with the ability to prioritise and meet tight deadlines whilst managing multiple priorities are essential.  Experience of working as an integral part of a successful team  Ability to deal professionally and compassionately with people with learning disabilities and their families / carers.  Experience of working with professionals from other agencies; developing and maintaining effective working relationships  Experience of working with people with a learning disability would be an advantage Attitudes  An unwavering belief and passion in inclusion and commitment to Talkback’s vision and mission.  Effective team player with a positive “can do” attitude but also able to work effectively alone.  Ability to give and receive feedback constructively and fully participate in support and supervision.  Willingness to learn new skills and adapt systems as needed as Talkback develops.  To be an active member of the team and promote the Talkback culture both internally and externally.
