Athletic Fee ($20/sport) Sudden Cardiac Arrest form HANG TIME AT EINSTEIN Hang Time Overview – Einstein is excited to be offering Hang Time for the first time this year. Hang Time is a new extensive after school activity program, which encompasses ALL of our after school activities and clubs! In order to participate in an Einstein club or after school activity, students must register for Hang Time. While Hang Time does not include athletic participation, it does include athletic event attendance. If students want to cheer on their classmates, they need to register for Hang Time! If students want to participate in an after school club, they need to register for Hang Time! Hang Time Information –This is a joint venture staffed by Einstein Middle School, The City of Shoreline, and the YMCA. Hang Time will begin on September 20th and meets Tuesday, Wednesday, and Thursday from 3:05 – 4:45 PM. To participate in Hang Time, students must submit a completed Hang Time Registration Form and have paid the ASB (Associated Student Body) fee. The ASB funds these clubs, activities and sports. In order to support these programs students are required to purchase an ASB card if participating in activities. Scholarships are available. See the attendance secretary in the main office for more information and registration forms. After-‐School Activity/Athletic Transportation – Activity buses for Hang Time participants run Tuesdays, Wednesdays and Thursdays and will be available regularly starting Tuesday, September 20th. Students participating in fall sports will have a special fall sports activity bus starting on September 8th (first day of practice). Buses leave the bus loading zone in front of the gym at 4:45 p.m. These buses are for athletes or students signed in through Hang Time ONLY! Athletic Event Attendance – Students who like to attend after school athletic events to cheer on their classmates must have a completed Hang Time registration form. Please turn in your 2016-‐17 Hang Time Registration Form ASAP if you would like to catch a game! Visit us online! http://schools.shorelineschools.org/einstein/ Immunization Information 201 th th New Requirement of TWO Varicella (Chickenpox) shots for all 7 and 8 graders! Washington state law requires that all children up to the age of 21 be fully immunized on or before the first day of school. Shoreline requires that students must have proof of th immunization or an approved waiver by the 20 day of school. If no proof/waiver by the th 20 day of school, the student may not continue to attend school until documents received. This means the school MUST have on file: § A Certificate of Immunization Form, OR § A Waiver Form exempting the student from these requirements for medical, philosophical or religious reasons. **NEW requirement for Waiver**: A change in the Law enacted in July 2011 requires the Waiver form for newly enrolled students and current students who have new immunization requirements to include a physician’s signature for those wishing to exempt for medical or philosophical reasons. A proof of membership in a religious group that does not allow a health care practitioner to provide medical treatment to a child is required for those claiming a religious exemption.
