Dear Madam, Dear Sir, As announced at the end of 2012 during the round table discussion dedicated to transport, the FANC is currently modifying its ICT system. To begin with, it has chosen to harmonise the licensing process and support it with a CRM system. In a next phase, this system will also support the inspection and auditing processes. Later on, a portal site will be developed in order to allow, among other things, the online submission of licences, prior notifications and monthly reports. This transition is planned for the near future. From then on, the various licences will have a different look and the prior notifications and monthly reports for transport will have to respect a very precise format. A new form will be developed later this year for the monthly reports. For the prior notifications, please find enclosed the new form along with instructions for its use. Please read it carefully and respect these instructions. This new rigid structure will allow the semi-automatic verification of prior notification data. As from 22 July 2013, prior notifications should be sent by e-mail, in this format, to the following e-mail address: [email protected] For every individual prior notification an e-mail has to be sent with the form attached. The licence number and the prior notification single reference number have to be stated in the e-mail subject. If the transport is carried out in convoy, it is allowed to send a single e-mail, provided that a duly completed standard form is attached for each individual vehicle. In this case, the single reference number for each document has to be mentioned in the e-mail subject along with the licence number. 2013-07-08-ML-6-1-20-EN 1/6 Other formats will still be accepted in the next 2 months. Starting from late September 2013, prior notifications sent to the FANC in any other format than this new format will no longer be handled. You must complete one form per licence and per vehicle. For transports in convoy, please make a separate form for each vehicle. The form consists of 2 parts: an upper part (fields 1 to 17) and a lower part (fields 18 to 33) for the transported substances. Field 1: enter the licence number. Please use the required format and the exact number as mentioned in the licence. Field 2: each prior notification must have its own unique reference number. You may determine this reference number yourself, the purpose is to be able to identify every single prior notification linked to the same licence. Field 3: please enter the number of revisions of the prior notification. This revision number must be updated upon each modification of the prior notification, so that it is clear how many times the prior notification has already been modified before. Field 4: date of transport/arrival on Belgian territory. In case of a maritime transport, enter the date of arrival in the harbour. Field 5: scheduled departure time of the transport or the hour of arrival on Belgian territory. In case of a maritime transport, enter the time of arrival in the harbour. Field 6: name and address of the company that submits the prior notification. This field must be completed only when the licensee does not submit the prior notification himself. Field 7: person who can be contacted (either from the company mentioned in field 6 or from the licensee) should the FANC require any additional information concerning the transport. Fields 8 and 9: e-mail address and phone number where this person can be contacted between the sending of the prior notification and the actual transport. Fields 10 and 11: please enter the name and address of the consignor and the name and address of the consignee if all radioactive substances mentioned in the form are transported from the same consignor to the same consignee. This means that the consignor and consignee are the same for all goods to be carried. This consignor and consignee have to be the actual consignor and consignee for the transport performed by the carrier. For the part of the transport by road to the airport, the consignee is the handling company. For the part of the transport from the airport, the consignor is the handling company. For the part of the transport by road or by rail to the harbour, the consignee is the dock operator (with indication of dock number or dock name). Idem for transport from the port. If the transport by road also goes through another country, please mention the names of the actual foreign consignor and/or consignee where applicable. Field 14: in case of a maritime transport, please mention the name of the ship. For any transport from or to the harbour by road or by rail, the name of the ship has to be mentioned as well. Field 15: sum of the transport indices of all packages on board of the vehicle. Field 16: sum of criticality-safety indices (CSI) of all packages on board of the vehicle. Field 17: any information that might in some way be useful. Fields 18 to 33: please describe all combinations of isotopes, their form, package type and package approval certificate. Please use a new line every time one of the features is different. Field 18: name of the isotope as stated in the licence. Only radioisotopes listed or described in the licence may be used. Field 19: the form in which the radioactive substances are to be transported: • • • sealed, unsealed or radioactive substances in special form. Field 20: if applicable, please mention the approval certificate for radioactive substances in special form. Fields 24 and 25: sum of the activity of the packages mentioned in field 23, expressed in Becquerel with the appropriate prefix. Please enter the number in field 24 and the unit with the correct prefix in field 25. Fields 26 to 29: name and address of the consignor for packages mentioned on this line if different from the previous lines. For non-Belgian consignors, please mention the country code before the postal code.
