This Statement of Purpose has been produced to meet the requirements of Regulation 12 of
The Care Quality Commission (Registration) Regulations 2009 and contains the information
as out lined in schedule 3 as amended by the 2012 Regulations.
Staffordshire and Stoke-on-Trent Partnership NHS Trust (the “Partnership Trust”) will keep
under review this statement of purpose and will make revisions as appropriate. These will
be provided to the Care Quality Commission within twenty eight (28) days any revisions
made.

Legal Status
Staffordshire and Stoke-on-Trent Partnership NHS Trust (the “Partnership Trust”) was
established on 1 September 2011, and was formed from the provider services of NHS Stokeon-Trent, NHS North Staffordshire and South Staffordshire PCT. In April 2012, the
Partnership Trust became the biggest provider of integrated health and adult social care
services in the UK when adult social care teams from Staffordshire County Council
transferred over to the Partnership Trust from 1 April 2012.
The Partnership Trust provides community health and adult social care services in
Staffordshire and health services in Stoke-on-Trent.
These services range from district nursing, health visiting, school nursing, managing four
community hospitals and health services in six prisons, through to providing very specialist
community care through talented and dedicated staff.
The Partnership Trust is the biggest integrated health and social care provider in the UK and
currently offers health and adult social care to adults living in Staffordshire (outside of
Stoke-on-Trent) through our integrated care locality teams. The 32 teams give people access
to a one-stop service for all their health and adult social care needs.
The Partnership Trust serves a population of 1.1 million people and employs in the region of
6,000 staff. It is located within the geographical boundaries of Staffordshire County Council
and Stoke-on-Trent City Council and contains a number of urban centres including Stoke-onTrent, Leek, Burton-upon-Trent, Cannock, Lichfield, Stafford, Tamworth and Wombourne,
although the geographic area is largely rural.
In addition the Partnership Trust provides sexual health services to people living in Leicester,
Leicestershire and Rutland, Shropshire and Telford and Wrekin.

Broadfield Ward
This ward has 23 beds and provides care for adult patients requiring
neurological rehabilitation. Patients present with a variety of neurological
diagnoses including stroke, Multiple Sclerosis, traumatic brain injury, brain
tumours and spinal problems.
Chatterley Ward
This ward has 25 beds; 20 intermediate care/ rehabilitation and complex
assessment beds and 5 palliative care beds.
Grange Ward
This ward has 32 dedicated step up beds to receive patients directly from
their own homes or usual place of residence or directly from the
Emergency portals within the local Acute NHS Trust.
Jackfield Ward
This ward has 20 rehabilitation beds; accommodating fractured neck of
femur pathway patients, complex assessment and rehabilitation.

Scotia Day Case Unit
The Unit consists of 10 trolley spaces. It provides care and treatment for
patients who have rheumatology conditions and patients attending for
supportive therapy treatments (e.g. blood transfusions). The Unit plays an
important part in clinical research trials.
Scotia Inpatient Ward
This ward has 10 beds and accommodates rheumatology in-patients.
Sneyd Ward: Stroke Rehabilitation Unit
This ward has 20 beds and provides specialist and co-ordinated
rehabilitation to adults following a stroke and is part of a wider stroke
service that includes an acute stroke unit based at the University Hospital
of North Midlands and the Community Stroke Team at the Haywood
Hospital.
The rehabilitation is provided by a range of specialist health professionals
which includes medical, nursing, occupational therapy, physiotherapy,
speech and language therapy and dieticians.
Specialist support from the Stroke Rehabilitation Unit includes:
 Expert care and guidance
 Use of a wide range of treatments and techniques
 Support for various physical, cognitive and psychological problems
that may arise as a result of a stroke
 Carer education and training to enable ongoing rehabilitation
following discharge
Hydrotherapy
The Haywood Hospital has a hydrotherapy pool available to selected
physiotherapy patients.
Hydrotherapy is a form of physiotherapy; patients are given an exercise
programme to do in a heated pool (35°C). The warm water helps the body
to relax and will help to relieve pain, which can help patients’ movements.
Exercising in water also helps strengthen muscles and increase joint
movement whilst they are supported and protected in the water.
Rheumatology Centre
The rheumatology centre at the Haywood Hospital currently welcomes
patients from North Staffordshire, Derbyshire and Cheshire; the catchment
area has a population of over 500,000 patients.
The centre deals with the investigation, diagnosis, management and
treatment of patients with arthritis and other musculoskeletal conditions
many of which are chronic conditions which require lifelong treatment.
The hospital is pioneering in many different aspects of rheumatology, the
first community rheumatologist was appointed here and the first
development of a computerised drug monitoring system was at also the
Haywood Hospital.
The rheumatology centre also has an active research team recognised

nationally and internationally for its work.
Walk in Centre
The walk-in centre is a nurse-led service which provides a range of minor
injury/ailment services, no appointment is needed and the facility is used
by approximately 55,000 patients a year.
The walk-in centre also provides a daily DVT service and a fracture clinic
service which operates three afternoons each week.

Minor Injuries and Illnesses Unit (MIU)
This unit provides the first point of call for assessment of minor injury and
illness; it is open between 8am and 8pm and provides a Nurse-led minor
injury and minor illness facility. Services provided include:
 Advice for bites, stings allergy related issues
 Advice/treatment for muscle and joint injuries e.g. sprains and strains
 Assessment, diagnosis and treatment for broken bones/fractures
 Emergency contraception and advice
 Treatment of ear, throat, urine, eye and some skin infections
 Wounds requiring insertion/removal of stitches and special care
 X-ray facilities

aims of the service:
Living Independently Staffordshire
Staffordshire and Stoke-on-Trent Partnership NHS Trust aim to deliver a high
quality community reablement service across Staffordshire through the Living
Independently Staffordshire (LIS) service.
The service forms part of the wider Community Intervention Service (CIS) and
works under the direct line management of the Integrated Service Managers.
The service is a multi-disciplinary team including, therapists, to support a
greater number of service users to reduce their dependency on long term
services and/ or 24 hour care. Our aim is to reduce admissions to hospital and
improve hospital discharges.
The service has been designed to increase peoples independence and
maximise their opportunities to live in their own homes and communities.
We aim to offer a person centred, flexible, high quality service designed to
support individuals to live independently within their own homes whilst
enjoying an improved quality of life.
The reablement service should lead to the following outcomes for people:




Following access to the service, no further support is required, or
Needs identified have been met and the service user will be
discharged from services, or
The service needs to be extended to achieve the full benefits from
reablement; this should be in exceptional circumstances only, or
Further services through an individual budget/ direct payment/
managed budget are required because there are on-going needs.
All service users will have their own person centred support plan, which will be
reviewed on a regular basis by CIS. All relevant individuals, including the LIS
service, involved in the support plan will be invited to contribute to the review
to ensure that the outcomes set are still appropriate and achievable.

Brighton House delivers a high quality, short term intervention and
residential respite service for a period of up to 6 weeks depending on
assessed needs.
Enablement is offered with the following eligibility criteria:
• The individual must be residing in Newcastle and or Staffordshire
Moorlands district.
• The individual must be over the age of 50
• The individual must be medically stable
• The individual requires a short term programme of enablement
and are likely to make progress
• The individual’s medical needs have been assessed as manageable
within the community and only requiring a minimal to a moderate
amount of district nursing input.
• The individual is in the early onset of dementia and is deemed of
being at risk of admission to long term care
• The individual can demonstrate consent to the admission and a
willingness to undertake any planned therapies
A full standard operating procedure and a service user guide is available
upon request.
The home intends to provide an enabling service within a residential
setting which is designed to support individuals to regain confidence,
remain independent and living in their own homes.
All health care needs will be met by local GP’s and district nurses along
with all other community based professionals. We will provide support for
individuals to access all appointments within the community including GP,
district nurse, dentist, optician and hospital appointments; we ask that
family members/friends/carers remain involved in assisting individuals to
attend these appointments. Medication is administered by trained staff,
supported by the GP and our local pharmacist. Support to enable
individuals to continue to control their own medication is encouraged. In
the event of an individual’s health deteriorating, a full assessment will be
carried out to establish the individual’s change in needs and how they can

be appropriately supported.
We understand that an individual’s independence is key to a successful
outcome of using our service. The home therefore has facilities throughout
to assist people accessing the service the best opportunity to achieve this
outcome. Our staff team are experienced and qualified to help individuals
maintain and develop skills that enable them to continue living
independently.
All care staff members have completed the Common Induction Standards
or are working towards completing The Care Certificate; this is followed by
a requirement to achieve a minimum QCF level 2 in Health and Social Care.
This is supported further by a specialised induction programme for all staff
that is relevant to this service area, with an ongoing training and
development plan for individual staff and the staff team as a whole.

Living Independently Staffordshire (LIS) Stafford delivers a high quality,
short term intervention and reablement service for a period of up to 6
weeks depending on assessed needs.

• and/or who meet the service eligibility criteria
We will encourage all eligible individuals with reablement potential to
access our service.
The service is available to all eligible adults with both physical and mental
health needs, including learning disability, and provides a wide range of
support including:
 Working in partnership with other professionals to provide support
with short term reablement/enablement goals where there is
potential for the person to improve life skills/maximise
independence in a sustainable manner. This will predominantly be
achieved in partnership with the therapists colleagues in the CIS
 Provide both hospital and home based assessments
 Rebuilding confidence following a stay in hospital or a
rehabilitation bed
 Crisis Intervention to prevent admission to hospital/ residential
care/palliative care
 Contribute to a holistic assessment of need including personal
care, health/ medication, cultural, social and housing needs
 Contribute to a timely review process ensuring positive outcomes
 Support with the smooth transfer of services at the end of the
reablement period to appropriate provider, where required.
 Support with signposting for those no longer eligible for services.
Service objectives for 2015/16 are to progress fully towards the Discharge
to Access model.
 Discharge to Assess Model – The Discharge to Assess Model is a
specialised service enabling individuals to benefit from returning
home after an acute hospital admission with the following

objectives:
 No loss of confidence by spending too long in hospital
 A reduction in unnecessary hospital delays; once an individual
is medically fit for discharge they should be discharged home
as soon as possible with sufficient support in place to meet

supported at home, when appropriate and allowing for
personal choice, for as long as possible
 Further decisions about an individual’s long term support are
made at home rather than in hospital
 Ensure that if reablement is needed an individual gets it;
