How to earn your vouchers
ELIGIBILITY
Students from the 1st through 8th grades are eligible to participate in the Mathletics program.
(One workbook per person.)
INSTRUCTIONS
1. Solve all the problems in this workbook and write your answers on the answer sheet provided.
2. Mail ONLY the answer sheet and voucher form to:
[Deadline: Friday, June 30, 2017]

3. Every student who turns in a completed answer sheet by the deadline will receive two (2)
vouchers to a 2017 regular season A’s home game. Good for select Monday – Thursday home
games (expires 9/6/2017).
TEACHERS, COACHES, AND YOUTH LEADERS:
• Please have one main contact per group/school.
• Main Contact: Mail in the class/school/team’s answer sheets. (Please have forms organized by
grade or participating classes.)
• Create a cover sheet that includes the main contact information (name, school, address, phone,
email) and the number of answer sheets enclosed. Please indicate the students who will be
paying for additional vouchers. Please keep the answer sheets of those paying directly behind
the cover letter.
• Please make a copy or keep track of your school’s participation list. The main contact will be
in charge of distributing all vouchers.
