Congratulations on reaching the USA Hockey Southeastern District Tournament. Statewide Amateur
Hockey of Florida and Carolina Amateur Hockey Association are honored to be hosting this year’s championships.
Very few athletes ever participate in a district tournament, and your players should be proud of their
accomplishment in qualifying for such a major sporting event.
The local organizing tournament committee is very excited and committed to hosting an exceptional
tournament that will be remembered by all in the years ahead. Enclosed is the tournament handbook, for your
convenience. It is necessary that each team complete certain preliminary procedures so that the tournament may
operate as efficiently as is possible. Detailed information concerning these procedures is found in this handbook.
Please study this information and the Rules and Regulations very carefully prior to your arrival at the tournament.
It is vital that necessary registration forms be completed online according to the schedule indicated in the
document “Important Dates to Remember.”

The information in this package will assist you in making your plans for this tournament. It is very
important you read and adhere to the guidelines and timeframes in order to assure a smooth running tournament
and a positive experience for everyone. A team representative must make arrangements to attend the pretournament coaches and credentials meetings. All information pertaining to actual tournament play, game
formats, start times, locker room assignments, etc. will be discussed and presented at these meetings.

IN MEMORY
Robert “Bob” McCaig
March 20, 1937 - January 22, 2016
The 2016 Southeastern District Tournaments are dedicated to the
memory of Coach Bob McCaig, former Southeastern District Coachin-Chief.
Bob McCaig played an instrumental part in the development of youth
hockey programs in North America for nearly 40 years. Born and
raised in Flin Flon, Manitoba, Bob won championship titles as a player
at the Midget and Adult levels before becoming a coach at Lambton
College in Sarnia, Ontario. In 1965, McCaig became Director of the
McCaig Hockey Academy in Sarnia, Ontario, where he developed onice training modules for participants ranging from beginners to elite
levels. In 1971, McCaig was appointed Administrator of the Ontario
Coaching Program, a position he held until 1987.
In 1988 McCaig began working with the Georgia Special Olympics,
organizing coaching and officiating clinics, and monitoring floor
hockey events for five years. Using this experience, McCaig became
the Southeastern District Coach-in-Chief and Administrator for USA
Hockey's Initiation Program.
As Coach-in-Chief, McCaig developed and implemented the USA Hockey Coaching Education Program throughout
the Southeastern District, which encompasses 11 states plus the District of Columbia. McCaig has also educated
and trained instructors, and was part of several Southeastern District Select Camps.
In addition, McCaig was named the Director of Hockey Atlanta, in which he developed on and off-ice programs for
both youth and adults and increased the number of coaching clinics annually from seven in 1987 to 100 in
2002. From 1985 to 1987, McCaig was the Director of Coaching Development for a local youth hockey association
in Port Huron, Mich., where he initiated a developmental program for both coaches and players titled "Hockey for
the fun of it." McCaig has earned his Level 5 (Masters) Certification in both the U.S. and Canada, and has also
served as an instructor for USA Hockey InLine since 1998.
Coach McCaig was the recipient of the 2003 Walter Yaciuk Award presented annually by USA Hockey's Coaching
Education Program to an individual who has made an outstanding contribution to the program during many years
of service as a volunteer.
Bob will be greatly missed by all his friends in the Southeastern District.

NIGHTLY TEAM COVERAGE SCHEDULE
USA Hockey’s policy requires that each team designate a person responsible for conduct and to supervise its
section of the hotel each evening between the hours of 9:00 PM and 1:00 AM, except when a team is playing.
Each team is required to fill out a “Nightly Team Coverage Schedule” form with the hotel upon registration to
assure that all players observe their team curfew. We strongly feel that this will create a better environment for
all participants during the USA Hockey Southeastern District Tournament.

COACHES AND MANAGERS INFORMATION
Player Eligibility
All players must be registered with USA Hockey and certified to a USA Hockey Team Roster (1-T) on or before
December 31 of the current playing season to be eligible for a District Championship with that team only.
Players who are citizens of another country and residing with a parent shall be considered eligible if the parent(s)
hold one of the following legal documents: Alien Registration Receipt Card, Permanent Resident Card or if the
parent(s) holds a non-immigrant temporary visa. See pages 123-124 of the 2015-2016 USA Hockey Annual Guide
for eligible visa categories or contact your USA Hockey Associate Registrar for more information.
Players who are citizens of another country and are in the U.S. without a parent must hold an Alien Registration
Receipt Card, Permanent Resident Card or an F. Academic Student Classification visa or J. Exchange Alien visa.
Credentials
The credentials review process includes mandatory pre-certification of credentials by USA Hockey Associate
Registrars serving the locale of the respective teams participating in the tournament and a final verification of
credentials at the credentials meeting at the tournament site.
All teams must provide copies of required documentation for credentials to the USA Hockey Associate Registrar
for their Affiliate Association within 24 hours of qualification for the tournament. Credential requirements for
teams participating in USA Hockey National Championships and District Playoffs can be found in the 2015-2016
USA Hockey Tournament Guidebook, N. Credential Requirements of Teams for Districts and Nationals, pages 2324. A representative from the team should contact the appropriate USA Hockey Associate Registrar to make
arrangements for a pre-certification review, in person or barring that, to have the required documentation for
credentials delivered to the Associate Registrar.
In addition, teams are required to present the required documentation for credentials at the District Tournament
credentials meeting. Failure to have a representative present at the credentials meeting will result in disciplinary
action and/or forfeiture of games for the team.

Credentials Meetings
A team representative must be present at the credentials meeting. Any errors or omissions not previously resolved
by the Associate Registrar will be clarified at the credentials meeting. All questions raised during the credentials
process must be resolved prior to the start of that team’s first game.
Each team is required to furnish the Credentials Committee the following items:

Tournament Guidebook)
(a) A completed Credentials Verification Form (form 1-C) and a USA Hockey Certified Roster Form (form 1-T)
created by registration software.
(b) A verification and affirmation that a review of each player’s birth certificate has been completed by a district
registrar or his/her designee. When no verification and affirmation is provided, a copy of government-issued
birth certificate, verification of birth or copy of a passport identification page for each rostered player is
required. Birth certificates are not required for players shown as verified on the official roster (Form 1-T) or
the credentials verification report (Form 1-C).
(c) USA Hockey Waiver and Release completed for each registered participant and team manager. (Not required
for participants who have registered online at usahockey.com).
(d) Sanctioned game scoresheets to verify compliance with 20/10 for Youth teams, 14/10 for Girls’/Women’s
teams, minimum number of games for eligibility rule for each team and rostered player.
(e) USA Hockey Consent to Treat Form for each rostered player, coach and team manager.
(f) Coaching credentials for each coach at the level mandated by USA Hockey as shown on the official roster
(Form 1-T), credentials verification report (Form 1-C) or required registration and coaching credentials
generated through registration software, whichever demonstrates the most complete and current
information.
(g) Completed Code of Conduct for each rostered player, coach and team manager.
(h) Non-U.S. citizens must meet proof of resident status eligibility and transfer from home country federation (if
required) as stated on pages 123-124 of the 2015-2016 USA Hockey Annual Guide.
(i) Other documents pertinent to team activities and player eligibility.

Coaching Requirements
All coaches must have current USA Hockey certification and the corresponding online age-specific module to
participate in the District Tournament. Additionally, all coaches must be SafeSport verified and have completed a
background screening in accordance with their Affiliate’s background screening policy. All coaches are expected
and required to follow all USA Hockey rules, including the helmet rule, during all pre-tournament and tournament
events. Non-compliance will result in a Discipline Committee hearing with a minimum one game suspension. A
complete listing of the coaching education levels and requirements can be found on pages 130-132 of the 20152016 USA Hockey Annual Guide, V. Coaching Education Program, B. Coaching Education Program Requirements.
All problems with Coaching Education Program certification must be settled with the local Association Coaching
Program Director prior to your arrival at the District Tournament.
Coaches Meetings
An opening meeting for all teams participating in the tournament shall be held prior to the start of play at a time
and place to be set by the on-site Tournament Director. All participating teams are required to have a
representative (coach and/or manager) present at such meeting. No one person may represent more than one
team at this meeting unless prior approval is granted by the Tournament Director. Failure to have a team
representative present shall result in the suspension of the head coach, as listed on the team’s official roster,
from that team’s first game of the tournament. See L. Opening Coaches/Managers Meeting, pages 17-18 of the
2015-2016 USA Hockey Tournament Guidebook. The team representative is required to inform the Tournament
Director of any suspension that must be served at the District Tournament. A member of the Tournament
Committee will be available to answer any questions.

The managers of teams qualifying for the tournament should complete the following pre-arrival steps within the
indicated time frames:

within 24 hours of your qualification for the tournament.
2. Complete the tournament registration forms on www.sedistrict.org by February 24, 2016.
3. As soon as your team qualifies for the Southeastern District Tournament make hotel arrangements.
Teams are not required to use any specific hotel and are free to make hotel arrangements on their own.
See page 14 of this Tournament Handbook for information on hotels.

(if desired). The cost of practice ice is not included in the tournament fee.

II Tournament.
This year’s host site is Carolina Ice Palace in Charleston, South Carolina. The rink and
volunteer staff are looking forward to providing you a great experience as you compete
for a chance to represent the District at Nationals this year.
Every player, coach, family member and volunteer that has helped make your season a
success should be extremely proud of you for being a part of this event and representing
their affiliate. Long seasons, heavy travel and huge sacrifices to allow our players the
opportunity to play the game we all love are well worth it and will provide a life time of

IMPORTANT DATES TO REMEMBER
The managers of teams qualifying for the tournament should complete the following pre-arrival steps within the
indicated time frames:
1. E-mail the following information to Ed Morris, [email protected], team name; local association;
Affiliate; manager’s name; home address, home and mobile telephone numbers and e-mail address;
within 24 hours of your qualification for the tournament.
2. Complete the tournament registration forms on www.sedistrict.org by March 2, 2016.
3. As soon as your team qualifies for the Southeastern District Tournament make hotel arrangements.
Teams are not required to use any specific host hotel and are free to make hotel arrangements on their
own. See page 22 of this Tournament Handbook for information on hotels.
4. Contact Matt Mons at the Carolina Ice Palace, 843-572-2717 Ext. 18, to schedule practice time(s) (if
desired). The cost of practice ice is not included in the tournament fee.
5. Deliver your credentials notebook to your USA Hockey Associate Registrar within 24 hours of your
qualification for the tournament.
6. Tournament entry fee of $1,200.00 payable to Carolina Amateur Hockey Association (CAHA) mailed or
delivered to Joe Spitz, 301 King Charles Circle, Summerville, SC 29485, no later than March 4, 2016.

John Crerar
John Crerar truly is the founding father of amateur hockey in the Southeastern United States. John organized the
first youth hockey program in the Southeast, the Capital Boys, in the mid-1950s. When the Amateur Hockey
Association of the United States (AHAUS, the organization known today as USA Hockey) was formed to promote
and regulate amateur hockey nationwide, John was instrumental in insuring that the Southeastern District was
recognized as an independent entity in the new organization's eleven district structure. He established the
Southeastern Amateur Hockey Association (SEAHA) as the Affiliate organization responsible for the development
of amateur hockey at all levels within the District. For a quarter of a century John Crerar served as President of
SEAHA, USA Hockey Southeastern District Registrar, and the Southeastern District's representative on the USA
Hockey Board of Directors. In 1999, John was elected to the position of Director Emeritus of USA Hockey. John
passed away on October 14, 2006. It is with respect, admiration and appreciation for John's service to hockey in
the Southeastern United States that one of the two District Tournament divisions at the Pee Wee level is
designated as the “Crerar Division.”
Terry Scott
As Vice President of the Southeastern Amateur Hockey Association (SEAHA) from 1995-97, Terry Scott served as
an articulate and enthusiastic spokesperson for amateur hockey in the Southeastern District. His many
accomplishments included drafting and securing adoption of the SEAHA by-laws that clearly defined the four zones
into which the Southeastern District was divided. These zones provided the basis for team representation in the
District Tournament Championship. Terry also provided counsel and support for the formation of an effective
administrative structure for the orderly development of hockey in Florida at a time when the sport was growing
more rapidly in that state than anywhere else in the United States. Tragically, Terry Scott’s tireless service to the
Southeastern District was cut short by his untimely death in 1997. The “Scott Division” bears his name in memory
of his dedication to making our sport better and more enjoyable for all.
