DIEGUEÑO’S 8TH GRADE KNOTT’S BERRY FARM TRIP PERMISSION SLIP
My son/daughter_____________________________________ (please PRINT full
name) has my permission to attend Knott’s Berry Farm, under school supervision, on
Wednesday, June 14th, 2017. I realize he/she will be boarding the bus at Diegueño
approximately at 8:45 A.M. and will return to Diegueño at 8:00 P.M. I realize that I
need to pick my student(s) up promptly at 8:00 P.M.
I agree to the following conditions:
1.
Students must come to school at their regular start time and attend classes until the buses arrive.
2.
Students who do not go to Knott’s Berry Farm must attend school for the entire day.
3.
Students cannot use previously bought Knott’s Berry Farm tickets or passes. This is a school package.
4.
Students must follow and obey all Knott’s Berry Farm and Diegueño Middle School rules and standards
of conduct. Students detained by Knott’s Berry Farm authorities for any infraction of park rules will be
released to parents only.
5.
Parents will be at DIEGUEÑO MIDDLE SCHOOL to pick up returning students by 8:00 P.M. It will be cold
outside when the buses return; tired children and supervising adults should not have to wait for parents
past 8:15 P.M. There are no phones to use!!!
6.
Students must ride the same bus both ways. There are no exceptions!!! (Please don’t ask).
7.
Ms. Butler, the Assistant Principal, will make the final decision as to who is eligible to go on this good
behavior school trip.

STUDENTS MUST HAVE THEIR ID TO PURCHASE THEIR TICKET AND THEY MUST HAVE IT TO GET ON THE
BUS OR THEY WILL NOT GO ON THE TRIP! (If you need an ID, bring $5.00 to the office to order one.
The orders must be placed by April 28th).

Students must bring in this permission slip, $50.00 ticket price and their student ID to the ASB OFFICE
during nutrition break, lunch or after school beginning Tuesday, April 18th through Friday, April 28th.
TICKETS WILL ONLY BE SOLD FOR 2 WEEKS ONLY: April 18th – APRIL 28th. NO EXCEPTIONS!
Checks should be made payable to Diegueño Middle School. This $50.00 includes entrance into Knott’s
Berry Farm, and round trip bus transportation. EXTRA money will be needed for lunch and souvenirs. If
for some reason your student does not make the trip, students that are eligible for a partial refund will
be refunded the week after the trip and sent home in the mail.
