Any 5 grader in the Duarte Unified School District (DUSD) who attends a field trip to City of Hope (COH) during the 20162017 school year
• Note: The poster contest is an optional activity.

To encourage 5 graders to use the information that they have learned, while participating in various activities during their
field trip to COH, in order to spread healthy messages on ONE of three important issues:
• Safety and Injury Prevention (The Brain Activity)
• Disease Prevention (Disease Simulation Activity)
• Reducing the Risk of Illness (Antioxidant Activity or The Human Digestive System Activity)
Choose ONE of the three issues listed above for your poster. Draw a poster that represents the issue you chose, using color
on a plain (unlined), white, 9” x 12” sheet of drawing pad paper, which can be obtained from your teacher. Include on the
poster a catchy slogan that complements the drawing. Make sure all words are spelled correctly. The poster should be turned
in to your teacher, along with a completed entry form taped to the back of the poster. No staples please.
• Note: Contest entries will NOT be eligible for judging without a completed entry form.

Only one contest entry per student will be accepted.
Students must work as individuals. No team work or help from other students or adults is allowed.
The poster entry must be ORIGINAL.
Posters must be drawn free-hand. No tracing or pre-fabricated drawings (i.e. computer-generated graphics, photographs,
collages, 3-D artwork, or trade-marked art) will be accepted.
5. The following media can be used- paint, crayon, markers, pens, and/or colored pencils.
6. Pop-ups and/or additional materials added to the poster paper are NOT allowed. The poster art must be drawn directly onto
the poster paper.
7. The poster must have a completed entry form taped to the back of it in order to be eligible for judging.
8. All entries will be judged on the basis of creativity, originality, and relevance to the field trip and contest themes. Judges’
decisions are final.
9. All entries become the property of City of Hope and will not be returned. City of Hope reserves the right to put entries on
display through any media.
10. Three contest entries from each school will be chosen as winners, and these students will be invited to attend another field trip
to City of Hope towards the end of the 2016 – 2017 school year.
11. An overall first, second, and third place prize will be awarded among entry winners from all elementary schools during the
second field trip to City of Hope. Overall winners will receive gift cards in the following amounts:

12. All contest entries should be turned into your teacher by the end of the school day on the following dates below:
• Maxwell Elementary School: Friday, December 1, 2016
• Beardslee Elementary School: Friday, December 16, 2016
• Valley View Elementary School: Friday, January 27, 2017
• Royal Oaks Elementary School: Friday, February 24, 2017
• Andres Duarte Elementary School: Friday, March 31, 2017
