Wellness At Work...And Beyond Program Overview Plan Description: The purpose of the Wellness Program is to guide employees in the use of effective health care strategies in order to achieve and maintain a healthy lifestyle. The Wellness Program is also intended to put in place health care utilization strategies that ensure efficient and cost effective use of healthcare services. Consistent implementation of such strategies by employees is expected to contain and even reduce overall health care costs for both the employees and the City of Malden. Phase I: Eligible Employees: New Employees New Enrollees to Health Insurance Employees Coming Off of Option B Plan In Phase I of the Wellness Program, the City will provide total financial incentives of $250 for each employee who completes an online Health Questionnaire to gather baseline information relating to an employee's weight, blood pressure, glucose, cholesterol and smoking status. Incentives will be placed on a Health Reimbursement Account (HRA) card to use for out-of-pocket medical expenses like co-pays, deductibles, prescriptions, emergency room visits, eyeglasses and dental work. Employees must complete an annual Health Questionnaire in order to receive their HRA incentive. Phase II: Eligible Employees: Employees on Phase I for 1 Year Employees Currently in Phase II In Phase II of the Wellness Program, the City will provide a total financial incentive or funds of up to $500 annually. Each employee who attains and thereafter maintains his/her milestones, as listed below, will receive $100 for each of the 5 milestones they meet. The funds will be placed into their HRA account to use toward out-of-pocket medical expenses. Blood Pressure Cholesterol Exercise Non-Smoking Weight Funds will be placed onto a HRA card to use for out-of-pocket medical expenses like co-pays, deductibles, prescriptions, emergency room visits co-pays, eyeglasses and dental work. Employees must complete an annual Health Questionnaire in order to remain in the Wellness Plan. Wellness At Work...And Beyond Program Overview cont... Option B: Employees who fail to make good faith efforts toward meeting milestones in the Wellness Program for two consecutive annual periods shall be withdrawn from either the Best Buy HMO Wellness 500 or the Best Buy PPO Wellness 1000 plans and shall be enrolled in either the Best Buy Option B HMO 1000 plan or the Best Buy Option B PPO 2000 plan, each that comes with an increase in deductible and premium as described below. Harvard Pilgrim HMO Best Buy 1000 plan. Similar in all respects to the Best Buy 500 plan except that deductible costs shall be $1,000 for an individual and $2,000 per family. Harvard Pilgrim PPO Best Buy 2000 plan. Similar in all respects to the Best Buy PPO 1000 plan except that deductible costs shall be $2,000 for an individual and $4,000 per family. Under Both Option B Plans: The employee's share of premium costs under Option B shall be 25%. Employees are withdrawn from the Wellness Program and shall be ineligible for re-enrollment to the Wellness Program for one full enrollment cycle. There is no Wellness Program associated with Option B. Employees are encouraged to remain in their current plan.
