Wellness Committee - Structure

Established 2006
1-2 representatives from each location sit on the committee, along with 3-4
Corporate staff, including a representative from the HR Department
There is a chairperson and an administrative assistant. Committee members
volunteer as Sub-Committee chairs as necessary.
There is a Wellness Bulletin Board set up at every location – used for all postings
relating to our employees well being. This board also contains the Mission
Statement and a group photo of the Wellness Committee
There are Wellness Committee shirts all members have – that are worn on
committee meeting days.
The committee meets every 6-8 weeks, utilizing Web-Ex to include all offsite
locations.

• 64 employees have participated so far in the “Get A Check-Up / Get
A Check” Program for plan year 2012.
