Tobacco smoking rates
across Australia, 2011–12

This report for the first time presents adult smoking rates at the
local level across Australia.
In 2011–12, 2.8 million adults smoked tobacco daily and smoking rates varied
across local areas.1 This report shows that the percentage of adults who
smoked daily varied four-fold across local areas, ranging from 6% in Sydney
North Shore and Beaches to 28% in Grampians. In four local areas, one in 10
adults or fewer were daily smokers.
Rates in some local areas were still relatively high as one-third of all local
areas had smoking rates of at least 20% – equivalent to at least one in five
people smoking daily.
Smoking is a major risk factor for many chronic conditions including coronary
heart disease, stroke and many cancers.2 Rates of smoking have been
declining for many decades. Latest national data shows 16% of adults
smoked daily in 2011–12.1

Smoking is still one of the largest single preventable causes of death and
disease in Australia2 and this represents an enormous social and economic
burden on individuals and the health system.3

Key findings
Variation across Australia
High rates of smoking occur in many local areas including
metropolitan, rural and disadvantaged communities.
Across all local areas, Grampians in regional Victoria,
had the highest smoking rate, with more than one in four
adults being current daily smokers, compared with one in
16 people (6%) in Sydney North Shore and Beaches.
To allow fairer comparisons between the Medicare
Local areas for which there were data, Medicare Local
catchments were grouped into seven peer groups
based on similar geographic remoteness and
socioeconomic status.
Across metropolitan areas, one in 10 adults were daily
smokers in the wealthiest inner-city areas (Metro 1
peer group) compared with one in six in lower-income
communities (Metro 2 and 3). In regional areas, one in six
(Regional 1 peer group) and one in five (Regional 2) were
daily smokers. In local rural areas, one in four adults were
daily smokers.
Variation across states and territories
The four local areas with the highest percentages of
adults who were daily smokers were in regional and rural
catchments in the following jurisdictions:
Across similar regional areas, smoking rates were
double in some places, indicating there may be
opportunity for reducing rates in these Medicare
Local catchments.
The ranges within each of the metropolitan, regional and
rural peer groups for rates of smoking were:
• Metro 1: from 6% for Sydney North Shore and
Beaches (NSW) to 14% for Northern Sydney (NSW)
• Metro 2: from 12% for Fremantle (WA) to 22% for
South Western Melbourne (Vic)
• Metro 3: from 14% for Western Sydney (NSW) to
22% for Macedon Ranges and NW Melb (Vic)
• Regional 1: from 14% for Hunter (NSW), Sunshine
Coast (Qld) and Barwon (Vic) to 21% for NepeanBlue Mountains (NSW)
• Regional 2: from 14% for North Coast NSW to 28%
for Grampians (Vic)
• Rural 1: from 16% for Far West NSW to 23% for
Townsville-Mackay (Qld)
• Rural 2: from 24% for Northern Territory to 26% for
Goldfields-Midwest (WA).

In 2008, the Council of Australian Governments
established a target national smoking rate of
10% by 2018, which Australia continues to
work towards.4
All four local areas with the lowest percentages of adults
who were daily smokers were in the wealthiest innercity suburbs in New South Wales and Victoria.
Although the percentage of adults who smoked daily
increased with geographic remoteness and lower
socioeconomic status, there was still considerable
variation across local areas with similar characteristics.

In 2011–12, four Medicare Local catchments (all in
Metro 1) had adult smoking rates of 10% or less:
• 6% Sydney North Shore and Beaches (NSW)
• 8% Inner East Melbourne (Vic)
• 10% Bayside (Vic)
• 10% Inner West Sydney (NSW).

About the data
The data were sourced from the Australian Bureau of
Statistics Australian Health Survey 2011–13. Participants
in the survey aged 18 years and over were asked whether
they smoked at least once a day. A current daily smoker
was defined as a person who smokes one or more
cigarettes, roll-your-own cigarettes, cigars or pipes at
least once a day. Chewing tobacco and the smoking
of non-tobacco products were excluded. For
