cancer, and obesity.

variety of programs and services that touch every
aspect of health. The resources and tools will be helpful
not only for decisions you make for yourself, but for
your family as well.

programs and services are not meant to disrupt your
work and home life, but to enhance them.

How do I participate?
Part 1
Complete Health Qualification Form
The Healthy Rewards Health Qualification Form must be
completed by a licensed provider. There are two ways
to complete this form:
 If you have had your annual physical exam with
your primary care physician between January 1, 2014
and December 31, 2014 you may use the results
from that visit to qualify. Contact your doctor’s office
to have them complete the form.

If you have not yet had your annual exam for the
2014/2015 plan year, schedule an appointment with
your physician and take the form with you. You may
have to wait for the lab results to come in to
complete the form.
Next step: Return the completed form to the address or
fax number listed on the form by December 31, 2014
Keep a copy for your records.
Note: Do not rely on your doctors office to fax in the
form for you. It is your responsibility to return the
completed form to PacificSource.
How to access the form: If you did not receive a
form in this packet during open enrollment, you can
find one on the employee portal.
Complete the HealthQuotient
The Health Quotient is an online assessment tool that
evaluates your current health, biometric measure and
lifestyle choices. This tool and resources is powered by
WebMD, a reliable source for health information.
You can complete the Health Quotient from any
computer that has Internet access.
 Visit www.pacificsource.com
 Log in to InTouch (right side of home page) putting
in your USERNAME. It will then prompt you to put
in your PASSWORD.
 Forgot your Username? Click on the “Forgot
Username” and follow the prompts.
 Forgot your Password? Click on the “Reset your
password” and follow the prompts.
 If you are still having issues logging in, contact
PacificSource at (888) 977-9299 or by e-mail at
[email protected].
New Users: If you have NOT already registered you will
need to click REGISTER NOW. Have your member ID
ready and access to your email. A PIN number will be
emailed to you in order to complete the registration
process.
 Click on HealthQuotient- here you will be re-directed
to the WebMD site. Click on Take HQ Now.
Note: if you have previously taken the Health
Quotient, your answers will be saved. You will only
need to update the areas that have changed since the
previous assessment. Please make sure to complete
the entire assessment and click the “submit “button,
to ensure you have completed it.
Your Privacy is Our Priority.
Your personal health information (PHI) is protected by the Health Insurance Portability and
Accountability Act (HIPAA) and will be safeguarded. Any use of your PHI is covered by Pacific
Source Health Plans Notice of Privacy Practices. Only your participation will be reported to
Fremont County to determine premium contribution amount.

PacificSource Programs and Services
Our Commitment to you
PacificSource is also committed to the health of our members.
The following is a list of programs and resources available to
you and all family members enrolled in the health plan.
24-Hour NurseLine
Most medical situations don’t happen during normal business
hours. PacificSource members have access to registered
nurses 24 hours a day, 7 days a week for non-emergency,
health related questions through our toll-free nurse line.

GlobalFit
Members have access to discounted gym memberships of up
to $120 per year through GlobalFit. Over 10,000 gyms,
including big chains and local favorites, are part of this
discount program. To get started, members only need to visit
GlobalFit.com/PacificSource, use their zip code to search for
local gyms, and sign up.
Health Assessment
Our Health Assessment, called HealthQuotient is available
online or via paper assessment to deliver health risk
information and tools to the individual participant. Individuals
receive an immediate comprehensive report upon completion.
This report demonstrates individual health risks as it relates to
diabetes, cancer, cardiovascular disease, and other lifestyle
choices. It provides helpful information on how to make
lifestyle changes to affect modifiable risk factors.
Health Manager– a comprehensive Online Wellness Portal
Powered by WebMD®, Health Manager is an online health and
wellness center with personalized information, health risk
assessments, and tools to help members make the most of
their health. Health Manager includes a tailored wellness
library for each individual, health tools, symptom and drug
interaction checker and much more.
My Health Assistant, located in the Living Healthy section of
Health Manager, can help your employees begin and maintain
a healthy lifestyle. Focus areas include: Exercise, Nutrition,
Smoking Cessation, Stress Management, Emotional Health and
Weight Management. These programs include customized
planners, trackers, and information to help each individual
meet their goals.
Hospital-Based Health Education Classes
Many local hospitals offer classes on topics to help patients
optimize their health. As part of their PacificSource coverage,
our members can participate in hospital-based health
education classes and receive up to $50 reimbursement per
class, series, or program, to a maximum of $150 per member
per plan year. Hospital based classes cover a wide range of
health topics, such as pregnancy and parenting, heart health,
weight loss, nutrition, and fitness. Members can check with
their local hospital for a list of eligible classes in their area or
visit our Web site for direct links to some area hospitals
offering classes.
Quit For Life® Tobacco Cessation
This program is available to members who want help quitting
tobacco and is presented by Alere in collaboration with the
American Cancer Society. The program includes phone based
one-on-one treatment sessions with a quit coach, toll-free
telephone access to the quit coaches for one year, coverage of
recommended nicotine replacement products, and a Quit-Kit
of materials designed to help stay on track between calls.
Recommended nicotine replacement products, such as an 8week supply of nicotine patches or gum, or a 12-week supply
of bupropion (generic of Zyban) or Chantix (when prescribed
by your doctor and with no pharmacy copay) are also included.

Weight Watchers® Program
As a part of the PacificSource medical coverage, members can
participate in Weight Watchers programs and receive
reimbursement. Members can choose between two
convenient options to learn how to lose weight and keep it off.
Members have the flexibility of going to meetings ($100
reimbursement) or following the plan completely online with
Weight Watchers Online ($40 reimbursement).
