• 2nd MOTION to amend the Hemorrhage Control protocol to apply tourniquet at least 3 in
above wound not over a joint and to add in an unstable scene or if the extent of the
wound cannot be fully assess in the field then the tourniquet should be placed proximal
and high & tight – Passed.
• 2nd MOTION to approve BLS acquisition of 12 lead ECG in field (as a regional option) –
Passed.
• Lengthy discussion on check & inject syringe pilot program and the fact that it is in direct
opposition with Policy 10-01 which requires that all BLS ambulance have AED and Epi
Auto-injector.
Medical Standards –
• Looking at reviewing the SEMAC Advisories.
• Item writing for exams is looking for physician participation.
• LB – there is an effort, by an agency appealing to the Governor’s office, to shorten the
BLS EMT course to 80 hours.
• Chair asking regions to look at standardizing protocols across the state.
• Protocols on website are now up-to-date.

Airway obstruction removal tool discussed as a replacement for normal suction devices, this
did not appear to be gathering any support.
PIER
Michael Reid – Awards submission date was previously changed from 6/1 to 5/1 due to need
to meet national deadline.
Education & Training
M. McEvoy for (M. Washington) – 2nd MOTION to rescind the requirement for CLI & CIC for a
NYS written exam score of 85 or greater within 3 years prior to certification or recertification –
Passed.
• Exam for instructors will start May 2016. Will have online registration for a 50 question
test, must get 70% or better to pass. 2nd MOTION to require as of May 2016 a onetime
passing score on the NYS CLI or CIC exam for initial or recertification – Passed.
• New CLI course approved, it will be 24 hours in length. Sponsors can charge and get
reimbursed. 2nd MOTION to approve the revised NYS CLI curriculum as distributed –
Passed.
• Transcare filed bankruptcy, VOM’s will be good for about 1 year.
• Planning more regional faculty roll outs by October 16th.
• Exam for instructors will start May 2016. Will have online registration for a 50 question
test, must get 70% or better to pass.
• Certification cards – BEMS is looking to get cards out within 3 weeks of the test.
• Total tested in 2015 was 19,375.
• CPAP is a regional option for BLS personnel; this is not a curriculum addition.
• Ambulance Epi project is in doubt at this time.
• Practical exam for spinal injury will remain the same. Spinal restriction is now part of the
curriculum and should be taught.
BEMS portion of NAMSE instructor course is now on-line. Anyone that would like to take the
course can take it.

bringing requests to the REMAC. Reminded all that requests that require REMAC approval
must come to the REMAC direct, not through the State, and must have the Agency Medical
Director signed on for it to be considered.

Q.I. Subcommittee – (JZ) – ALS database was updated as of 3/29 and there are currently 825.
Had 60 referrals to QI with no trends. Baldwin FD was approved to use Alpine software.
Currently there are 25 agencies approved to use ePCR and 17 are actively using them.
Vendors that are approved in Nassau are: EMS Charts, Red Alert, Image Trend, Rescue
Medics, iPCR, Sansio, and Fire Rescue.
Protocol Subcommittee – (JR) – no meeting.
Credentials Subcommittee – (DK) – no meeting.
Policy Subcommittee – (NK) – no meeting.
Regional Trauma Advisory Committee – (JJ) – Dr. Axelrad – question on action on their
request regarding prehospital trauma notifications. Policy subcommittee will meet to move this

Medication Shortage – Drug shortages list was provided, it is getting shorter, now down to
just 1 page.
Voting Member renewals – Response to our letter about intentions was received from Dr.
Safford. Motion by Kirschen/Zaso to recommend to the REMSCo that Dr.Safford be
reappointed to voting membership on the REMAC – Passed unanimously.
ALS Protocol Revisions – Question on III.C, does not include IN for Glucagon and III.Y why
not Ativan or Versed. On III.C – discussion on IN administration, no FDA or manufacturer
approval or recommendation for administration via IN – MOTION to amend protocol III.C to
add IN administration of Glucagon (caveat that Agency Medical Directors have the option of
not allowing there personnel to use IN administration) – Passed with 2 NO votes. On III.Y –
discussion on why Ativan and Versed are not included in this protocol MOTION to add
Ativan & Versed as “or” in Medical Control Options – Passed unanimously.
Stroke data sheet – pre-arrival info for EMS to transmit to MedCon/Ops for retransmit to
receiving hospital – thanks to Winthrop Hospital for sharing this form.
New Business
• Inter-regional Protocol Agreement (Nassau, NYC, Suffolk) – the existing agreement was
formalized into a written agreement for all regions to sign as a BEMS requirement for the
regions.
• EMS physician credentials in Nassau – referred to Credentials subcommittee for further
review for on-scene physicians.
• Discussion on the Check & Inject Pilot Program and the need for the REMAC to participate
in the program MOTION by JKugler/Kirschen to submit certification to program that Nassau
meets their requirements and applications will be submitted – Passed 1 NO vote. It was
pointed out that all agencies that participate must still carry EpiPens as per DOH policy.
Additional lengthy discussion was held.

NYC ALS Protocols for approval:
There was discussion about use of IN glucagon including the off label use of glucagon. This is already a
Western Region (AKA-Collaborative Protocol) SEMAC approved route. The NYC changes were
passed including IN glucagon.
Suffolk Co EMS protocols:
Nor-epi: create a standard dose rather than weight based dosing-changed.
Dextrose, d50 and d10- give glucagon first then start the d10 drip
Medical control options-It was asked why not provide drug dose ranges for the docs: this was mentioned
that there is a specific reason for this, not to limit the physicians in their practice of medicine.
Additionally, the catastrophic use of medical control options has been removed. So the medic will not be
able to use without contact to MD
Old business:
Review of policies/advisories from SEMAC: two long outdated SEMAC advisories: 97-01 on
hemophilia and 97-02 recommending biphasic AEDs were voted on to be removed.
Lee Burns: A Rural NY local supervisor is looking to shorten the training requirements for the EMT
training program. She brought this up to the NAEMSE to be sent out to other states. Feedback is that
the EMT-B is “too long” nationally. NYS teaches to the national guidelines, not a particular time limit.
But the classroom time ends up being 180 hours. Students are tested on the national requirements. She
mentions that this is a financial issue for the more rural ems crews and counties.
Collaborative Protocols, state wide protocols: All regions are being asked to look at these protocols and
adopt them to standardize EMS care in NYS. Even for NYS- Nassau (yes our region) is being asked to
do a side by side comparison of their protocols and the collaborative protocols. ****
TK Placement: There was extensive discussion on the protocol. What was resolved is the followingPlacement should be 3 inches proximal to wound, unless situation demands high and tight, eg mci,
urgent placement in unstable environment, unable to determine location of bleeding on extremity. The
protocol on the state website will be re-worded.
Lastly, if tk placed or if pt meets criteria- transport to NYS regional trauma center.
There was a discussion to update the part 800 supply list since it was last modified when created in
1987. This is to include the tourniquet.
Informational: –NYS BNE controls narcotics here, but other states do not have a BNE type of
enforcement and thus allows controlled substances to be used under standing orders.
Life-vac device discussion: voted not to be approved for ems use as a suction device. It is able to be
used by non-ems units.
Dr Henry mentioned that the body traditionally approves standards not devices.

SEMAC meeting notes
Subcommittees:
Ed and Training-Mike McEvoy, CLI and CIC INSTRUCTOR exams are set to begin in May. IF YOU
ARE EXPIRING IN MAY YOU NEED TO TAKE THIS EXAM INSTEAD OF THE STATE EMS
WRITTEN EXAM. First will be rescinding requirement for 80% exam score for 3 years initial and
recert exam. Will be sent to SEMSCO for vote. New requirement: for May 2016 cli and cic will need
to get a passing score on this credentialing exam. The rest is the same for all nys ems exams a 70% is
passing. With the ability to take second time, if failed second time a remediation process will be done.
New CLI curricula-approved by the committee will be asked for approval. Lean initiative-Bureau will
begin to print cards in house to expedite process of cards.

landmark year for EMS testing in 2015 with a total of 19,375 exams given, the highest since 2011.

CPAP skill is a regionally approved option, and the training needs to be done under a regionally
approved program, initially by an ALS CIC then once up and running a certified BLS CIC in the
future. As a body in Nassau, we need to re-visit the approved programs to make sure there is a NYC
CIC involved in the initial credentialing. I’m not sure we did this last meeting.
Medical Standards- 5 items
1 and 2) NYC and Suffolk protocol changes were accepted
3) Motion to rescind 97-01 and 97-02 (hemophilia and biphasic aed) MMA
4) Language changes to hemorrhage protocol-passed
5) BLS acquisition of 12 lead for transmission to physician-passed
Bureau Report-Lee Burns;
1) Transcare bankruptcy-NYC, Westchester, Putnam, Duchess Counties have rallied to recover
from this loss of 911 units. “None of the ambulance services are too big or too small to fail!!”
Continuity of Operations plan was reviewed as a result of this closure. This is happening all over
the state, and some agencies are closing without a plan.
2) Bureau staffing: down 2 ems reps in NYC, she discussed how bems is financed as a special
revenue account. Main office is also short staffed.

3) Ems not providing emergency departments with pcrs is a statewide concern
4) no pre-notifications with trauma notifications per STAC- or units are making notifications at
the back door of the facility, not giving hospital enough time to mobilize resources needed before
ems unit arrives.
5) Parts: 800-5 and 800-12 changes (narcotics and reciprocity) regulations are nearing
completion
6) blood regs: 2 apps received, 1 approved will try and standardize review process to make the
approval process more streamlined and easier for all (applicants and reviewers)
7) BLS protocols on web page are most current since 2005. Don’t panic if it does not look
pretty, but they are there. The ones that have not been updated appear as in past.

SNCH, Albany Med Center, Vassar Brother’s-provisional level 2 Poughkeepsie . Good Sam in
Suffern has been DE-designated as provisional.
Check and inject: update- 236 agencies enrolled 42 career 112 volunteer 84 remainder mix. 17
REMACs, 1245 kits placed in service. 0 utilization called in so far. Agencies are looking to get rid of
epi pens- but this is a demonstration project, and agencies must maintain the epi-pens until
demonstration is complete. So do not remove from budget. Lee Burns also asked that the epi pens
not get taken off the ambulance remaining as a backup.
