MEDiC Outreach Program MEDiC Clinics 4234 Health Sciences Learning Center 750 Highland Avenue Madison, WI 53705 [email protected] (608) 265-4972 (608) 265-8560 (fax) Enhancing Education Serving Others Thank you for your interest in donating medications and/or medical supplies to the MEDiC Student-Run Free Clinics. Your support is vital for the continued success of MEDiC and is greatly appreciated. Due to an agreement with UW Hospital and the University of Wisconsin School of Medicine and Public Health, MEDiC clinics operate using a strict formulary. Therefore we are only able to accept medications that are currently on our organization’s formulary. We also accept a variety of medical supplies. Please review the attached list that describes: 1) The complete formulary list for medications we are able to accept 2) A list of medical supplies that we accept To ensure the safety of our patients, MEDiC does not dispense any medication that is near to, or past its expiration date. We ask that any medications donated have at least 3 months before their expiration date. We are also unable to accept drug samples of any kind per UW Hospital policy. We are able to accept brand name versions of any of the drugs on our formulary. Thank you so much for donating to MEDiC clinics. Your generosity helps uninsured patients receive the care they need and invests in the continued success of our organization and Madison as a community. Thank you as well for understanding our limitations in regard to these donations. We look forward to working with you in the future.
