Contest starts at or about March 23, 2015 and ends on the earlier of the close of business on or
about June 15, 2015 or when all Free Pizza for LIFE contest peel tabs (a “Peel Tab”) have been
distributed. Contest is open to all legal residents of Ontario who have reached the age of majority.
Gino’s Pizza® customers who purchase a slice of Gino’s Pizza® may receive a specially marked
pizza slice box or tray with a promotional Peel Tab at participating Gino’s Pizza® locations or you
may receive a Peel Tab with no purchase by mail. There is one (1) Grand prize of Free Pizza For
LIFE available to be won, consisting of one (1) large three (3) topping pizza per month for twenty
(20) years. Gino’s Pizza ® reserves the right to direct the winner of the Grand Prize (Free Pizza
for LIFE) or any other food prize to a Gino’s Pizza® location of its choice and such prizes will be
for pick up of the prize only, as delivery charges are not included. Approximate retail value of the
Grand prize is $3854.00. Winners must correctly answer a mathematical skill-testing question.
Odds of winning the Grand prize are 1:250,000. Number of prizes diminishes as claimed. For

How to Participate: From the open of business on March 23, 2015 to the earlier of the close of
business on June 15, 2015 or when all Free Pizza For LIFE contest peel tabs (each a “Peel Tab”)
have been distributed (the “Contest Closing Date”) at participating Gino’s Pizza® locations.
Gino’s Pizza® customers who purchase a slice of Gino’s Pizza® may receive a specially marked
pizza slice box or tray with a promotional Peel Tab at participating Gino’s Pizza® locations in
Gino’s Pizza® For LIFE promotional contest (the “Contest”) and while supplies last. Limit of one
(1) promotional Peel Tab per person per purchase. All prizes must be redeemed in accordance
with these rules on or before July 15, 2015. Open to residents of the Province of Ontario only who
have reached the age of majority. Void where prohibited. No Purchase Necessary. To obtain a
randomly selected promotional Peel Tab without purchase, send a request with a self-addressed
envelope bearing sufficient postage to Gino’s Pizza® for LIFE Contest, “Gino’s Pizza® Pizza For
LIFE Promotion, Promotions Department, 15 Brownridge Road, Unit 5, Halton Hills, Ontario,
L7G 0C6”., for timely processing and return. Limit of one self-addressed stamped envelope and
request per outer envelope with sufficient postage. A single, randomly selected Peel Tab will be
returned in the self addressed, stamped envelope while supplies last. All No Purchase Necessary
requests must be postmarked no later than the Contest Closing Date and received by June 15,
2015.
Prizes. There will be 250,000 Peel Tabs available at the outset of the Contest. Upon receipt of
the Peel Tab, pull the marked area, and if it reads: (a) “You WON Free Pizza for LIFE”, you may
be eligible to win Free Pizza for LIFE, as such prize further described below; (b) “You WON a Trip
for 2 to Las Vegas” you may be eligible to win a trip for 2 to Las Vegas, Nevada; (c) “You WON
Free Pizza for a Year”, you may be eligible to win Free Pizza for Year, as such prize is further
described below; (d) “You WON a 48” Flat Screen TV”, you may be eligible to win one (1) 48” Flat
Screen TV; (e) “You WON a Mountain Bike”, you may be eligible to win one (1) Mountain Bike;
(f) “FREE LARGE 3 Toppings PIZZA ”, you may be eligible to win one (1) Large Pizza with 3
Toppings; (g) “FREE GARLIC DIPPING SAUCE” you may be eligible to win one (1) Garlic
Parmesan Dipping Sauce; (h) “FREE SLICE” you may be eligible to receive one (1) FREE slice; (i)

(with any purchase over $10.00)” you will receive $1.00 off from Gino’s Pizza®, when you use
this Peel Tab and your purchase is over $10.00; (k) “WIN 6 Garlic Bread Sticks” you may be
eligible to receive one (1) FREE 6 Garlic Bread Sticks;

The Grand Prize of Free Pizza for LIFE consists of one (1) large three (3) topping pizza per month
for twenty (20) years and each Secondary Prize of Free Pizza for a Year consists of one (1) large
three (3) topping pizza per month for one year. Gino’s Pizza ® reserves the right to direct the
winner of the Grand Prize (Free Pizza for LIFE), the Secondary Prize (Free Pizza for a Year) or any
other Food Prize to a Gino’s Pizza® location of its choice and such prizes will be for pick up of the
prize only, as all prizes do not include transportation to/from such Gino’s Pizza® location or
delivery charges.
Each trip for two (2) to Las Vegas, Nevada consists of round trip flights for two (2) two (winner
plus one (1) guest) between Toronto Pearson International Airport and Las Vegas, Nevada.
Winner and guests must travel to Las Vegas on the same flight/itinerary with the same service.
Prize also includes one (1) standard dual-occupancy room (or similar) for l days/l nights stay;
prize package includes cost of room per night (maximum l nights) plus applicable taxes. All other
expenses not listed, including, but not limited to, airline seat assignment and baggage fees, meals,
incidentals, tips, telephone calls, etc. are the responsibility of the prize winner and their guest.
Services required for passengers with special needs are not included in the package and may not
be available. Travel must be completed by December 23, 2015. Approximate retail value of prize is
$2,200.00 CAD. Actual value may vary based on location and rates available at the launch date of
the Contest. Any penalties associated with changes made to any reservation following
confirmation of booking will be the responsibility of winner and their guest. Blackout dates or
other restrictions may apply and all reservations are subject to availability. Prizes do not include
transportation to/from the airport in Toronto or Las Vegas and winners are responsible for travel
insurance if desired. Prize is non transferable and has no cash value. The Grand Prize winner and
his/her guest (who must be the age of majority in Ontario) must have, and are responsible for
obtaining, all required travel documents, including passports, health and travel insurance. If the
Grand Prize winner cannot use or chooses not to use, for any reason, any portion of the Grand
Prize, that portion of the Grand Prize will be forfeited and no substitution or other compensation
will be awarded.
The number of prizes available to be won will be reduced as they are won during the Contest.
The approximate retail values set out above are in Canadian Dollars and are approximate values
at the time of printing these rules. All estimated retail values do not include applicable goods and
services tax, provincial sales tax or other use or consumption taxes.
To Redeem the Grand Prize or a Secondary Prize. To redeem the Grand Prize or any
Secondary Prize, complete the claim form (located at any participating Gino’s Pizza® store),
correctly answer, without assistance or mechanical aid, the mathematical skill testing question
and send it together with the original potential winning Peel Tab to: “Gino’s Pizza® Pizza For
LIFE Promotion, Promotions Department, 15 Brownridge Road, Unit 5, Halton
Hills, Ontario, L7G 0C6”. DO NOT SURRENDER YOUR “GRAND PRIZE” WINNING
PEEL TAB TO A COUNTER ATTENDANT OR ANYONE ELSE–MAIL IT TO THE
OFFICIAL INSTANT WIN ADDRESS.
It is strongly recommended that all materials be photocopied prior to mailing and that all
material be sent using insured Registered Mail with a receipt of mailing. Please note proof of
mailing is not proof of receipt. Once the Peel Tab has been verified you will be declared a winner,
subject to completing the Declaration and Release and correctly answering the skill-testing
question. All materials, including, but not limited to, potentially winning Peel Tab, claim form,
and a correctly completed mathematical skill testing question, must be postmarked by no later
than 5:00pm (EST) June 15, 2015 or the prize will be forfeited in its entirety. Please allow 6 to 10
weeks for delivery. Prizes are not redeemable for cash. No refunds, exchanges or time extensions
will be permitted. Not valid in conjunction with any other offer. This offer is not for resale. No
copies or facsimiles will be accepted. In order to be declared a winner, a potential Grand Prize or
Secondary Prize winner must also sign a standard declaration and release form confirming
compliance with the contest rules, acceptance of the prize as awarded and releasing Gino’s Pizza
Inc. and their franchisees, their respective dealers, partners and affiliates, their respective
advertising and promotional agencies, parent companies, subsidiaries, shareholders, directors,
officers, governors, employees, representatives and agents (collectively, the “Released Parties”)
from any and all liability.
In order to claim the Grand Prize or any Secondary Prize, the winner must have reached the age
of majority in the Province of Ontario on or before the Contest Closing Date. If any individual
under the age of majority wins a prize, the prize will be awarded to the parent, legal guardian or
other authorized representative of such person who must accept the prize on the individual’s
behalf.
To Redeem Food Prizes and Special Offers. For each of the following, present the original
potential winning Peel Tab at the following locations by no later than close of business on July 15,
2015:
a) Free Large 3 Topping Pizza - Present the potentially winning Peel Tab to any
participating Gino’s Pizza® store to redeem the special offer.
b) $1.00 off any food purchase of over $10 at Gino’s Pizza®– Present the Peel Tab to
any participating Gino’s Pizza® store to redeem the special offer;
c) Free Garlic Parmesan Dipping Sauce – Present the potentially winning Peel Tab to
any participating Gino’s Pizza® store to redeem the special offer.
d) FREE Slice– Present the potentially winning Peel Tab to any participating Gino’s Pizza®
store to redeem the special offer.
e) Free Garlic Bread – Present the potentially winning Peel Tab to any participating Gino’s
Pizza® store to redeem the special offer.
f) Free 6 Garlic Bread Sticks – Present the potentially winning Peel Tab to any
participating Gino’s Pizza® store to redeem the special offer.
All Food Prizes and Special Offers are not redeemable for cash. No refunds, exchanges or time
extensions will be permitted. Not valid in conjunction with any other offer. This offer is not for
resale. No copies or facsimiles will be accepted.
Prior to being declared a winner, each participant must first correctly answer, without assistance
or mechanical aid, a mathematical skill testing question and, together with the “winning” Peel
Tab, present it to a participating Gino’s Pizza®, as the case may be, for verification.
General.
1.
The Contest is open to all legal residents of Ontario who have reached the age of majority,
except employees (and persons with whom they are domiciled) of Gino’s Pizza® or Gino’s Pizza®
franchises or their respective advertising and promotional agencies. All decisions of Gino’s
Pizza® are final and binding and Gino’s Pizza® shall not be held responsible for any accident,
negligence, printing, administrative or other errors that may arise or occur in connection with the
Contest. Gino’s Pizza® reserves the right to modify, amend or terminate this contest with no
obligation or liability, subject to applicable law. Unclaimed prizes will not be awarded.

Prizes must be accepted as awarded, are non-refundable and non-transferable, cannot be
converted to cash, have no cash surrender value, except that Gino’s Pizza® reserves the right, at
its sole discretion, to substitute a prize with one of equivalent or greater value for any reason. If
necessary, Gino’s Pizza® has the right to direct the winner of the Grand Prize (Free Pizza for
LIFE), or the Secondary Prize (Free Pizza for a Year) or the Food Prize (Large 3 topping pizza) to
a Gino’s Pizza® location of its choice and such prizes do not include transportation to/from the
designated Gino’s Pizza® location or delivery charges. Taxes, if any, are the winner’s
responsibility. No correspondence will be entered into except with the selected entrant. By
entering this Contest, the entrants consent to the use of their name, city of residence, video image,
and/or photograph in any related publicity or Internet posting without compensation or
notification.

Gino’s Pizza® is not responsible for lost, late, stolen, misdirected, mutilated, illegible or
postage-due entries. All Peel Tabs and entries become the property of Gino’s Pizza® upon receipt
and will not be returned. No responsibility will be taken for any failure of any entry to be received
by Gino’s Pizza®. Entries are subject to verification and will be declared invalid if they are
illegible, mechanically reproduced, mutilated, forged, falsified, altered or tampered with in any
way. Peel Tabs that contain printing, production, typographical, mechanical or other errors are
automatically void. Liability for such promotional Peel Tabs is limited to replacement with
another Peel Tab, while supplies last.

Gino’s Pizza® reserves the right at its sole discretion to disqualify any individual who
tampers with or attempts to tamper with the entry process. CAUTION: ANY ATTEMPT BY AN
ENTRANT TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE
CONTEST MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN
ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM
ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW INCLUDING
CRIMINAL PROSECUTION.

By entering this contest, entrants agree to indemnify and hold Gino’s Pizza®, Gino’s
Pizza® franchises, and their respective advertising/marketing agencies (the “Released Parties”)
against any and all liability, damages or causes of action (however named or described) with
respect to or arising out of (i) entrant’s participation in the contest or (ii) the receipt or use of any
prize awarded herein. None of the Released Parties shall bear any liability whatsoever with
respect thereto.
6.
If, due to a printing, production or other error, more than the specified number of prizes
are printed or claimed, only the number and type of prizes as contemplated by these rules will be
awarded. Upon discovery of such error, all prize fulfillment will cease and a random draw from
amongst all eligible claims received by June 15, 2015 and will take place on July 15, 2015 before
the end of the business day, to select entrant(s) to receive the remainder of the prizes specified by
the rules. Remaining prizes will be awarded to the first entrant(s) selected who correctly answer a
mathematical skill testing question and comply with the contest rules.
7.
To receive a list of winners of the Grand Prize and Secondary Prizes, send postage paid
self addressed stamped envelope within three (3) months after July 15, 2015, to “Gino’s Pizza®
Pizza For LIFE Promotion – List of Winners, Promotions Department, 15 Brownridge Road, Unit
5, Halton Hills, Ontario, L7G 0C6”.
8.
This Contest is governed by the laws of Ontario and the laws of Canada applicable
therein, and is subject to all applicable federal, provincial and municipal laws and regulations.
