Responding to a Jury Summons
Directions: Read.
If you receive a jury summons in the mail, you need to respond to it within 10 days.
The jury summons has a juror affidavit questionnaire attached to it. You need to:
1. Detach the juror affidavit questionnaire from the jury summons.
2. Complete the questionnaire (write in the required information).
3. Mail the questionnaire back in the envelope provided.
4. Follow the instructions on the jury summons to either
a. call the phone number on the date listed (listen to instructions), or
b. report to the court (the address and map are on the jury summons).
