For the protection of all pets in our care, boarders must have current immunizations (see above) and be free of
internal and external parasites. Any pet that is not up-to-date will be immunized and fleas, ticks, and

If your pet becomes ill while boarding, a doctor will treat your pet promptly and will perform only tests
immediately needed. If there are any other recommendations for your pet’s care, a doctor will discuss these with
you. We will contact you if a life-threatening condition develops, or you may indicate below if you wish to be
called for other problems. You will be responsible for all additional medical fees.

We require notification of additional boarding needed beyond the date specified below. If you fail to inform us
and do not pick-up your pet within 7 days after the designated date of discharge, we will consider your pet
abandoned and begin legal procedures. You will still be financially responsible for all fees incurred.

You may leave personal belongings with your pet, but we cannot be responsible for loss or damage of these
items. These items must be and will be marked with permanent marker for identification.
If you have special concerns, please communicate these with a doctor or animal care nurse.
Multiple Pet Boarding

disagreements or injury to my pets. Should injury occur I will be responsible for all charges. The charges may
include an exam, medications, surgery, etc.
