1. Go to website www.hamariladli.org and click on center registration on the top.
2. Enter the PIN provided to you and click continue.
3. You will get the following screen. Fill all the details of your center.
4.Enter Your Equipment and Radiologist details on the following link.

4. Enter Username and password and click Submit you will get confirmation
message of center registration.
5. Go to website www.hamariladli.org and click on Login.Enter your username and
password to fill online F-Form.
Login with your User
Name And Password
6.Once you login with your user name and password you can able to access site And this is
your Dashboard/first page.
It is displaying the total number of F formsubmitted online. It automatically calculates
total F forms from 1st April to till today andrecords of the current month.
Hamariladli Patient Registration
1. After Login Click on Registration _ Patient Regn
2.You will get the following Patient Details form. Fill the details
and submit the form.
3. You will get the following screen or
Click on Registration _ FFormRegn.
4. Either search the patient or click the check box of the patient for
which you want to fill the F Form and click on the F Form Button
above.

5. You will see the F Form as follows with all details of the
patient. Fill the remaining Form and Submit it.
