Team Carnival Game/Activity Registration The Relay For Life of Western Berks is proud to once again host "Hope Carnival" as our on-site fundraiser. As always, teams are not required to participate in the on-site fundraiser, but we sure hope you will! In order to minimize the leg-work our teams need to do, the Committee has already done the research and has outlined it in this document. Teams wishing to participate in Hope Carnival should complete the last page of this form and return it at the March team captain meeting. Forms will be processed and game selections announced at the April meeting. Thanks for everything you do to make our Relay the best in Berks County!!! By pooling our resources and purchasing in bulk, we will be able to offer the best possible prizes at the lowest possible cost. Teams who wish to participate in the prize tent are requested to do the following: Issue tickets (in lieu of prizes) to the winners of their games. Work in the prize tent for a minimum of 2 hours (schedule will be created and you will have the opportunity at the May team captain meeting to sign up for the hours you want to work). Contribute a percentage of each dollar raised to cover cost of prizes in the Prize Tent. The MAXIMUM expected prize cost is twenty five cents/dollar raised. Therefore, for every dollar you raise, your team will bank at least seventy five cents! We will only charge the exact cost of prizes and that will be determined at the conclusion of the carnival. The accounting team will deduct the prize cost from the money that your game has banked throughout the carnival. 2 Team Name: Team Captain: Check all that apply: My team achieved a 2011 Team Fundraising Club Level (Circle level) Bronze $1,500 Silver $3,000 Gold $5,000 Platinum $10,000 Sapphire $15,000 Diamond $20,000 My team was registered on or before the September 21, 2011 Kick-off.
