THE NEW HAMPSHIRE LOTTERY COMMISSION® $25,000 MEGA MAKEOVER GAME TICKETS SECOND-CHANCE DRAWING RULES PROMOTION PERIOD The New Hampshire Lottery Commission is conducting a second chance promotion (the "Second Chance Drawing"), in connection with the New Hampshire $2 “$25,000 Mega Makeover” scratch ticket (the "Game Tickets"). The promotion period begins March 28, 2016. The Game Tickets may continue to be sold after the Second Chance Drawing period ends on Thursday, August 25, 2016. GENERAL RULES Any non-winning ticket for the $2 Mega Makeover instant scratch game (NH Lottery game number 1260) may be entered for one of two (2) Second Chance Drawings as detailed below. Eligibility for participation in the Second Chance Drawing is dictated by the rules and laws of the New Hampshire Lottery Commission. To enter the Second Chance Drawing, visit the New Hampshire Lottery website, www.nhlottery.com/THD (the “website”). To enter the Second Chance Drawings, players must enter the serial number of a non-winning Game Ticket in the space provided on the website. There are no restrictions as to the number of entries of non-winning Game Tickets, but each individual ticket can only be entered once. Entries are to be filed in only one individual’s name. You may submit more than one entry per Second Chance Drawing. However, prizes are limited to one per person and/or e-mail for each Second Chance Drawing. Odds of winning any prize in either Second Chance Drawing shall vary depending upon the number of entries received for the Second Chance Drawing. Entries submitted after the deadline for the first Second Chance Drawing shall be placed in the next Second Chance Drawing. Entries received after the final entry deadline shall not be eligible for any Second Chance Drawing. The first Second Chance Drawing will take place Thursday May 26, 2016. The second Second Chance Drawing will take place Thursday August 25, 2016. Second Chance Drawing dates are subject to change based on ticket sales, distribution, and claims. Winners shall be notified by the New Hampshire Lottery of their winning. Winner names, town, state, and prize won shall be posted on the New Hampshire Lottery website as well as the Second Chance Drawing website. ENTRY RULES To be eligible to enter the $25,000 Mega Makeover Second Chance Drawings, you must register on the website and enter your name, mailing address, city, state and ZIP code, phone number and a valid email address and check the box to certify that you are at least eighteen (18) years old and understand that all Game Tickets, transactions and winners are subject to these Rules, the rules and regulations of the New Hampshire Lottery as applicable, state laws, and the Terms of Use and Privacy Policy located on the website. The website is the only method of entry into this Second Chance Drawing. Entries sent via any other method (US mail/courier, drop-off etc.), shall not be opened and/or reviewed and shall be disqualified. You must enter the serial number from your non-winning $25,000 Mega Makeover Game Ticket for each entry. The serial number is the 13 digit number that is found on the bottom front of your non-winning Game Ticket in the scratch-off area. The New Hampshire Lottery Commission nor its promotional partner Alchemy 3 shall be responsible for data entry errors by entrants. The serial number from any non-winning Game Ticket may be entered only one (1) time. Second Chance Drawing members do not have to retain the Game Tickets they submit as proof of eligibility to claim their prize. The entry deadline for each of the two (2) Second Chance Drawings is 11:59 PM ET on May 23, 2016 for the first drawing and 11:59 PM ET August 22, 2016 for the second drawing. Entries from the first drawing will be discarded after the first drawing and will not be eligible for the second drawing. Only tickets entered from 12:00 AM ET on May 24, 2016 until 11:59 PM ET on August 22, 2016 are eligible for the second drawing. All Second Chance Drawing entries are subject to validation by the New Hampshire Lottery Commission or its designee and may be disqualified if eligibility requirements are not met. Game Tickets determined to be in a stolen/missing inventory status or not part of a valid sold, activated or promotional inventory status shall be disqualified. The New Hampshire Lottery Commission and/or its administrators, at their sole discretion, may disqualify an entrant, winner, or designee from participating in any aspect of the Second Chance Drawings if they deem or suspect that such person has engaged in or has attempted to engage in any of the following: (a) Act in violation of these Official Rules or the Terms of Use or Privacy Policy; (b) Damage, tamper with or corrupt the operation of the website; (c) Act with intent to annoy, harass or abuse any other person; (d) Use an automated bot, macro, script or other robotic, mechanical, programmed or automated devices to submit data to the website; (e) Repeatedly attempt to submit multiple entries; (f) Behave in any unsportsmanlike, inappropriate, uncooperative, disruptive, fraudulent, potentially fraudulent, or unusual behavior or activity; or (g) Participate in any activity deemed to be generally inconsistent with the intended operation of the Second Chance Drawings. WARNING: ANY ATTEMPT BY A PERSON TO DELIBERATELY DAMAGE OR CORRUPT ANY WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SECOND CHANCE DRAWING IS A VIOLATION OF CRIMINAL AND CIVIL LAWS. The New Hampshire Lottery will pay 25% Federal Withholding Tax) Actual drawing dates are subject to change at the sole discretion of the New Hampshire Lottery Commission. There are no cash options available on any prize won. All prizes will be paid in the form of The Home Depot® Gift Cards. Federal taxes estimated at 25% of the value of each package that is valued at $600 or greater will be borne by the New Hampshire Lottery Commission, whose agents will send winners IRS Forms 1099 in January of the year following the respective receipt of the awarded prize. Any other taxes or fees will be responsibility of the prize winners. WINNER NOTIFICATION Winners will be notified by mail using the contact information provided via the website. Winners will receive an initial letter and claim form from the New Hampshire Lottery. Winners will be required to complete a New Hampshire Lottery Commission Prize Claim Form as well as provide copy of photo identification and proof of social security number (photo identification and proof of social security number on prizes of $600 and greater only) and return same within 30 (thirty) days of the forms being mailed from the Lottery. Failure to comply will result in the prize package being forfeited and the prize will be awarded to the first available alternate winner. Once claim forms are received back from the winners and processed, Alchemy 3 will send the prize within 30 days. All prizes will be sent via USPS with tracking so delivery can be confirmed. The NH Lottery offers no warrantees or guarantees of product quality and that availability and quality of the prize and its conformance to advertised representations is the sole responsibility of the promotional partner, Alchemy 3. The NH Lottery shall not be responsible for any prizes lost, damaged or stolen during shipment, pickup or use. CHILD SUPPORT INTERCEPT – Pursuant to RSA 284:21-v, the New Hampshire Lottery Commission is required to review all lottery prize claims subject to federal tax reporting with a child support database provided by the Department of Health and Human Services (DHHS). In the event that a winner of any prize package described above owes child support, the winner shall have 21 days from the draw date to settle the total arrearage due as determined by DHHS to the DHHS, after which settlement the winner will be awarded the prize. Failure to settle any arrearage shall result in that winner’s prize package being forfeit and the package will be issued to the first available alternate winner. The original winner(s) owing Child Support shall receive no other prize or compensation from the New Hampshire Lottery Commission relating to this game. Page 4 of 4
