ERRAZO Mr. Errazo is a 64 year old Filipino National with 35 years electrical trade experience. Mr. Errazo previously held a temporary electrician license (TE-247) tied to Mepco Ltd. but had to leave the island. Mega Systems Ltd. now wishes to employ him and is asking for his license to be tied to them, they submitted along with his application a cover letter, two reference letters, copy of his passport, their Trade & Business License, list of their licensed employees and their license numbers to support the application. 5. 2 DANIEL B.HOFFMAN Mr. Hoffman is a 51 year old American National with 30 years electrical trade experience. Mr. Hoffman previously held a temporary electrician license (TE247) tied to Electra-Tech Services with whom he is no longer employed. Barnes Electrical Services now wishes to employ him and is asking for his license to be tied to them, they submitted along with his application a cover letter, one reference letter and a copy of his passport to support the application. 4 5. 3 BRIAN J. Corporate Electric Ltd. is applying for a temporary electrician license for Mr. Owens and submitted along with his application a cover letter, resume, copy of his Canadian Electrician License, their Trade & Business License, list of their licensed employees and their license numbers to support the application. 5. 4 JEREMY J. EASTON Mr. Easton is a 39 year old Canadian National with 19 years electrical trade experience. Corporate Electric Ltd. is applying for a temporary electrician license for Mr. Easton and submitted along with his application a cover letter, two reference letters, resume, copy of his Canadian Electrician License, their Trade & Business License, list of their licensed employees and their license numbers to support the application. 5. 5 GARFIELD O. GORDON Mr. Gordon is a 40 year old Jamaican National with 22 years electrical trade experience. Mr. Gordon previously held a temporary electrician license (TE-247) tied to NEC Ltd. but was rolled over and to leave the island. True Power Electrical now wishes to employ him and is asking for his license to be tied to them, they submitted along with his application a cover letter, resume, his Jamaican Electrician License and various certificates to support the application. 6.0 MASTER ELECTRICIAN 7.0 ELECTRICAL ENGINEER 8.0 EXAM/LICENSING MATTERS 9.0 ELECTRICAL CONTRACTORS’ LICENSE 9. 1 CHURCHILL & NADINE BROWN (BROWN’S ELECTRICAL COMPANY) Mr. & Mrs. Brown submitted along with their Electrical Contractors’ application, a cover letter stating that Mr. John Richards (E-033) will be the responsible electrician for their company, two reference letters and a copy of their Trade & Business Licence to carry out the business of Electrical Contractor to support their application. 5 Decision from October 13, 2015 (meeting 10/15 item 9.1) It was resolved to invite Mr. & Mrs. Brown and their responsible electrician Mr. Richards to appear before the Electrical Board of Examiners at their meeting on November 10, 2015 at 3:00 pm. to address their concerns with regards to the above matter. Decision from November 10, 2015 (meeting 11/15 item 9.1) It was resolved to refuse the application, for the following reason: 1. There is no responsible electrician for the company as Mr. Richards is employed by Crawford Electrical and therefore is unable to provide the required level of oversight as required by Regulation 60(2). Mr. & Mrs. Brown has submitted a new application along with a letter stating that Mr. Edlin Moore (E-046) will be the responsible electrician for their company and is asking the Board to once again consider their application. Decision from December 17, 2015 (meeting 12/15 item 9.1) It was resolved to invite Mr. & Mrs. Brown and their responsible electrician Mr. Edlin Moore to appear before the Electrical Board of Examiners at their meeting on January 12, 2016 at 2:30 pm. to address their concerns with regards to the above matter. Mr. & Mrs. Brown and Mr. Edlin Moore are scheduled to appear before the Electrical Board of Examiners at 2:30 pm. Decision from January 12, 2016 (meeting 1/16 item 9.1) It was resolved to defer the application to permit the applicant to provide the Electrical Board of Examiners with details of an individual who fulfils the requirements of the Electricity Regulations 60(2) (2011 Revision), as the Electrical Board of Examiners was not satisfied that Mr. Edlin Moore would be able to adequately perform the duties as manager/supervisor due to his current employment arrangements. Section 60(2) “Qualification for an Electrical Contractor’s licence shall be that the applicant is the holder of, or whose business is managed and supervised by the holder of, a valid licence under regulation 61(1)(a), (b) or (c) for a period exceeding two years immediately preceding the date of the application and who satisfies the Board that he is a suitable condidate for such licence. Mr. Brown submitted a letter in response to the Board’s decision stating that Mr. William Dawson would be the responsible electrician for their company. A round robin was conducted with the Board via email on January 15, 2016 regarding the letter, and it was resolved to invite Mr. & Mrs. Brown and Mr. William Dawson to appear before the Board at their meeting on February 9, 2016 at 3:00 pm. to address their concerns with regards to the above matter. Mr. & Mrs. Brown and Mr. William Dawson are scheduled to appear before the Electrical Board of Examiners at 3:00 pm. 6 9. 2 BURNS G. RANKIN (RANKIN’S ELECTRICAL SERVICES) Mr. Rankin submitted along with his Electrical Contractors’ application, a cover letter and a copy of his Trade & Business Licence to carry out the business of renovations, maintenance services & fire alarm Installations to support his application. 10.0 OTHER MATTERS 10. 1 RAYMOND FISCHER Block 39E Parcel 8 Below is a memo from Senior Electrical Inspector, Shondel Bodden in regards to the above. Director of Planning Cayman Islands Government Department of Planning REF: Electrical Board of Examiners - Mr. Peter Dandie of PETEX Electrical LTD. Electrical Contractor License No.ECL-002, Electrician License E-090 Synopsis On Thursday 29th of January 2015, while in the District of North Side I arrive at the premises of Mr. Raymond Fischer, Block 39E Parcel 8, No.1482 Rum point Road and observed a significant amount of electrical works being carried-out without first obtaining approval as required per regulations 29 of the Cayman 7 Islands Electricity Regulations (2011 Revisions). According to the building contractor Mr. Kel Ebanks (9362454) the electrical works were being carried-out by Mr. Peter Dandie of PETEX Electrical LTD. I contacted Mr. Dandie while on site and request for the electrical work to discontinue until an application was submitted and approved. Mr Dandie was also asked to address an unsafe working environment which exited as a result of an electrical panel having exposed live parts thus subjecting construction personnel to unnecessary shock hazard. (see photo No.3 below). In addition to the panel interior being exposed there was no GFCI protection provided for receptacles being used by construction personnel. Though Mr. Dandie was aware since January 2015 that an electrical application was required to be submitted for approval, it is important to note that he subsequently submitted an application on the 08th of June 2015 which merely seeked approval to upgrade the service from 200 amps to a 300 amp 120/240V 1P due to three additional loads being added (see attach approved document). Because the application did not accurately represent all of the changes made to the building’s electrical system, the service upgrade was reviewed approved and subsequently authorized for connection without requisite inspections being carried-out to a substantial amount of new electrical works. See email below sent to Mr. Dandie on Thursday 15th October 2015 after it was determined that approval had been obtained based on inaccurate information. In considering the extent of works carried out by PETEX Electrical LTD without first obtaining required approval, the company submitting an electrical application for approval which inaccurately represented works actually performed and, occupants likely being subjected to an unsafe environment due to requisite inspections not being performed, I consider it prudent in forwarding this matter to the EBE for consideration in accordance with PART IV of the Cayman Islands Electricity Regulations (2011 Revisions). New A/C Decision from December 17, 2015 (meeting 12/15 item 10.1) It was resolved to invite Mr. Dandie to appear before the Electrical Board of Examiners at their meeting on January 12, 2016 at 3:00 p.m. to address their concerns in regards to carrying out electrical works without a permit contrary to Sections 29(1)(a) of the Electricity Regulations (2011 Revision). Section 29(1)(a) “Save as provided in Regulations 42 to 44, no licensee shall, until he has obtained a building permit(a) do any electrical wiring work. Mr. Dandie is informed that his matter will be considered in relation to Sections 25 and 28 of the Electricity Regulations (2011 Revision), which states: Section 25 “If it appears to the Board that a licensee has failed to comply with any condition imposed in respect of any license issued under these regulations, the Board may suspend or revoke the licensee’s license. Section 28 “A person who carries out any electrical works or installations which constitutes a danger to human life or safety commits an offence and is liable on 9 summary conviction to a fine of ten thousand dollars or to imprisonment for three months.” Decision #1 from January 12, 2016 (meeting 1/16 item 10.1) It was resolved to re-invite Mr. Dandie to appear before the Electrical Board of Examiners at their meeting on February 9, 2016 at 2:30 p.m. to address their concerns in regards to carrying out electrical works without a permit contrary to Sections 29(1)(a) of the Electricity Regulations (2011 Revision). Upon stopping at the site I spoke with Mr. Burt Delson (3243434) and Mr. Sheldon Brown who appeared to be carrying out electrical works without supervision or being licensed as required per Regulations 63 and 59 of the Cayman Islands Electricity Regulations (2011 Revisions). Mr. Delson mentioned that he work for Mr. Peter Dandie of PETEX Electrical LTD and that Mr. Brown was only on site visiting him since he works for Mr. Fitzroy Solomon of Fitzroy Maintenance. I tried contacting Mr. Dandie while on site however I was unable to make contact until the following day of which he confirmed being responsible for the installation and that no approval was obtained. In addition to unlicensed workmen carrying-out electrical works without first obtaining approval as required per regulations 29, an unsafe working 11 environment exited as a result of the electrical panel being energized and having exposed live parts (see photo No.2 and 3 below). As a result of life-safety concerns and the scope of works carried out without first obtaining approval, Mr. Dandie was instructed to discontinue further works until an application is received by the Department of Planning and approval obtained. In considering the extent of works carried out by PETEX Electrical LTD without first obtaining required approval and the company permitting unlicensed workmen to work in an unsafe environment, I consider it prudent in forwarding this mater for consideration in accordance with PART IV of the Cayman Islands Electricity Regulations (2011 Revisions).
