RE: LU Johnston Estates Phases I, II & III Cleveland, TN Project No. W-16437 This letter acknowledges receipt of six (6) sets of plans for the above re^encea project,'Wl i have completed our review of this project and have found it to be apparently satisfactory,CiO plans have been stamped "approved for construction". This letter with the enclosed construction plans bearing our stamp constitutes approval by the Commissioner of the Tennessee Department of Conservation and Environment for construction of water facilities. Approval for water facilities is granted in accordance with the Water Quality Control Act of 1977 and Regulations of the Water Quality Board. Approval expires one (I) year from the stamped approval date unless construction is either underway or complete. Approval of these construction documents should not be construed as a permit for any activities related to this project. Activities which may require a permit under the WQC Act and Regulations include, but are not limited to, the following: stream bank vegetation removal; creek crossing(s) for equipment or utility lines; construction within twenty (20) feet of a stream bank; or construction in or near a marshy area or wetland. The Natural Resources Section of the Division of Water Pollution Control (615/532-0625) should be contacted for determinations regarding an NPDES permit or an Aquatic Resource Alteration Permit (ARAP) for those activities that may result in degradation of waters of the state. Cleveland Utilities is retaining two (2) copies of the plan for our records located in the Johnston Estates subdivision file and forwarding one (1) copy to the Department of Environment and Conservation, Municipal Facilities Section of the Division of Water Supply in Nashville. Three (3) copies of the approved plan are attached. A set of the approved plans must be kept on the job site during construction. Cleveland Utilities requires that prior to the beginning of construction work our office must be notified so inspection of the project can be maintained. Box 2730 • Cleveland, TN 37320-2730 • (423)472-4521 Deviations from the approved plans must be submitted to Cleveland Utilities for approval prior to any changes being made. A licensed contractor in Tennessee must complete all waterline construction work. The contractor must be approved by Cleveland Utilities prior to the beginning of any work. All work is to be done in accordance with the latest set of Cleveland Utilities Water and Sewer Specifications. The developer/owner of all subdivisions in Cleveland, Bradley County Tennessee has the responsibility of locating and coordinating installation of water and sewer lines with other utilities until all utilities are fully installed. No meters shall be set in a subdivision/development until all fire hydrants and post hydrants are plumb with break aways at proposed ground level (i.e. fire hydrants should not lean to one side and not stand too high or low). All gate valve boxes should be level with final elevations. The developer/owner must submit as-built drawings, on reproducible mylar film, of the completed waterline construction. Failure to submit as-built drawings shall be cause to refuse to accept application for service to any lots in the development. After the proposed waterline is installed and tested (Pressure & Bacteriological), it is the developer's responsibility to contact Cleveland Utilities to pay the prevailing fee for Long Side meters. This fee covers the materials for water meters to be set on lots located across the road from the new waterline. This work will be done by Cleveland Utilities' forces. Adequate time must be allowed for these "Long Side" meter sets to be installed in advance of the final paving of the roadway.
