AUDITION ENTRY FORM
This Audition Entry Form is required for bands auditioning for the Hayward Area Recreation and Park
District’s (H.A.R.D.) 52nd Annual Battle of the Bands. Auditions are required for all potential performers and
will be held on Saturday, January 10, 2015 at the Matt Jimenez Community Center, 28200 Ruus Rd; Hayward.
REGISTRATION INFORMATION
 A copy of each band and crew members photo I.D. must accompany this Audition
Entry Form at the time of registration or the form will not be processed.
 Registration information is on reverse side.
 Audition times and details are not finalized until you are notified by H.A.R.D.
 Applications will be accepted until thirty-five (35) bands are registered.
 NON REFUNDABLE $20 APPLICATION FEE MUST BE SUBMITTED WITH APPLICATION
AUDITION INFORMATION




Each band will play a maximum of twelve (12) minutes.
All band members must have their own equipment.
All bands must provide their own P.A. for the auditions.
All individuals involved in the performance (including up to two crew members) must
attend the audition.
 No guests are allowed at the auditions, only performers and crew members.
AWARD INFORMATION
 Prizes are different year to year. Past prizes have included studio time, photo
sessions, music equipment, advertisements, etc.
 First place winners will receive individual plaques; the second and third place
winners will receive a single band plaque. The top three will all receive special
prizes based on the final rankings. All bands participating will receive a
certificate of participation.
IMPORTANT INFORMATION
 Finalists must send one representative to attend the Battle of the Bands Information Meeting on Thursday, January
29th, 2015 at 7:00pm at Chabot College Performing Arts Center, 25555 Hesperian Blvd. Hayward.
 All of the finalists and crew are required to attend Battle of the Bands rehearsal on the day of “The Battle”
 All Bands chosen for the Final Show will required to Purchase 40 tickets at a discounted price in advance.
 All decisions by the Hayward Area Recreation and Park District and their representatives are final.
 Failure to comply with any and all of the rules will result in forfeiture of a bands involvement in this event.

from responsibility for loss of or damage to our equipment during auditions, rehearsals or the Battle of the Bands
performance. All band and crewmembers have read and understand all the information on the reverse side of this
Audition Entry Form. The bands are responsible for obtaining the legal right to perform copyrighted material and are liable
for any and all royalty costs associated with copyrighted material. H.A.R.D. can use the band names, addresses, photos
and other video images for promotional use. All band and crewmembers understand that a video production company
working with H.A.R.D may contact you.
