A student may request 5 Kinesiology credits for the following:

Participation for three seasons on a CHS interscholastic sports team
Participation in two full seasons of Dance
Three full seasons of Sideline Cheer or Competition Cheer
Two sports in one year for Classes of 2017, 2018 & 2019
This credit can be applied once only (manager positions excluded). A student is still required to take Health and
one additional Kinesiology course to fulfill the graduation requirement of 15 credits. An Academic Reflection*
paper must be turned in with this form.
STEPS TO COMPLETE THE REQUEST:
1. Meet with your CHS College Counselor for her to verify your participation in CHS sports.
2. Complete the Academic Reflection paper.
3. Turn in this form (on back) and Academic Reflection to Mr. Cushing by May 1.
