Federal Tax Forms
Filing deadline is Monday, April 18, 2016!
Introduction
The Internal Revenue Service (IRS) is reducing the number of printed forms and instruction booklets
offered to libraries since there has been an increased popularity of electronic tax filing. Federal tax
forms are no longer mailed to individuals and businesses.

Which tax forms will be available at Cleveland Public Library Main Library and Branches?
The federal tax products Cleveland Public Library is receiving:
-Forms: 1040, 1040A, 1040EZ
-Instruction Booklets: 1040, 1040A, 1040EZ
-Publication 4604 (English/Spanish): Use the Web for IRS Tax Products & Information
-Publication 1132: Reproducible Copies of Federal Tax Forms & Instructions
-*Publication 17: (1 Reference Copy is available at each Branch and the Main Library)

Ordering IRS Forms
Ordering by Phone
Patrons may call 1-800-Tax-Form (829-3676) to order most current and some past year tax products.
Ordering Online
Patrons may also fill out an online form to order most current and some past year tax products.
Steps to Order Forms Online:
1) Go to: https://www.irs.gov/

4) Select Forms and Publications by U.S. Mail
5) Place your form in the search box near the bottom of the page.

Taxpayer Advocate Service (TAS) of the IRS
TAS can be reached at 877-777-4778.
The TAS offers free help with IRS problems that you can’t resolve on your own.

State of Ohio Tax Forms
Ohio Department of Taxation:
State of Ohio tax forms are no longer mailed to individuals and businesses. The Ohio Department of
Taxation is decreasing the number of tax products they send libraries since less than 10% of all printed
returns were actually being filed with the state of Ohio last year.
Cleveland Public Library Main Library and Branches are receiving the major State of Ohio forms and
instruction booklets.
Major Developments in Ohio Tax Form Products this Year:
- The Ohio IT1040, IT1040EZ, and amended IT1040X forms have been consolidated into one Universal
IT1040 Tax Return.
- The Ohio SD 100 and Amended SD 100x school district forms will be merged into one Universal SD
100 Tax Return.

Cleveland Public Library Main Library and Branches are receiving CCA forms.
The city of Cleveland uses the CCA (Central Collection Agency) form.

Tax Preparation Assistance
Cleveland Public Library
Free tax preparation help is offered at select Cleveland Public Library Branches provided by volunteers
from CCA, AARP, and/or VITA/EITC.
Documents required: Current photo ID, Social Security Cards for taxpayer and all dependents, proof of
current health insurance, last year’s tax return, and W2 and 1099 forms for the 2015 tax year.
Appointments are required.
