What is Deliberate Walking?
Overuse Injuries
By: Cory Rogotzke DPT
By: Becca Boeving BS, AT, ATC
Do you suffer from chronic foot, ankle, knee or hip pain? Was there no specific cause of this pain? Then a solution for
you could be changing how you walk. Since you have learned to walk, you have done so in a heel-to-toe pattern. To you and
everyone else this is natural, but is it? This pattern originated when people started wearing shoes. The sole necessitated a heel-totoe walking pattern. This pattern could be leading to an assortment of musculoskeletal problems.
Deliberate walking aims to change this pain cycle and decrease/eliminate
lower extremity musculoskeletal issues. It is a form of walking with a forefoot
strike followed by early heel push off. This creates a bounce like gait. Dr.
William Lee, a surgeon in Chelsea first brought this concept to our clinic. The
initial contact forefoot strike engages the functional mechanism of the plantar
fascia, not allowing for any arch collapse. With initial contact at the heel this
does not occur and subsequent arch collapse does, leading to possible ailments
over time. During the heel push off phase of gait all muscles of legs and core
are activated. This occurs for a longer duration during deliberate walking
therefore leading to increased lower extremity muscle strength, compared to
heel-to-toe gait.
Like the rest of physical therapy, deliberate walking is not a quick fix. It takes a concerted effort over time to make the
lasting changes. Take it from me, I have had plantar fasciitis in my right foot for the past year. I have been focused on changing my
gait using deliberate walking and have currently reduced pain with walking by 90%. So if you suffer from lower extremity pain and
have not had success with any other treatment, try deliberate walking and get your bounce back!

Spring sports have kicked off and the overuse injuries are already in full swing. Shin splints, various forms of tendonitis, and
muscle strains are a few problems athletes struggle with during this time. Some ways to prevent these injuries are to ensure a
proper warm up before beginning a practice or game. One key role in a warm up should be stretching. Stretching should be
included in a warm up and a cool down. Keeping muscles loose before and after participating in sports will help keep these
injuries at bay. Proper technique when performing specific tasks such as pitching or running will put less strain on surrounding
muscles that can be injured when poorly performed.
If athletes find themselves affected by an overuse injury, they should
ensure they are doing sufficient stretches before and after athletic activities.
Stretching the correct muscles in an appropriate manner is important.
Athletes should reflect on proper technique to make sure they are using
correct mechanics. Next, athletes should ice following practice or a game.
Lastly, athletes should seek exercises relevant to injury in order to strengthen
their muscles to decrease pain and increase function. Doing exercises that
target specific muscle groups can make it easier to do the task at hand,
ensure the proper muscles are being used throughout specific tasks and in
return the athlete can perform more sufficiently.
