Guide to Completing Application Form for Naturalisation
(Form 8 - Person of full age)
This Guide and the accompanying application form are for those people who wish to become
Irish citizens by naturalisation. You should familiarise yourself with the statutory criteria for
naturalisation before you complete this form.(Information Leaflets available on request or on
our Web site (www.justice.ie)
All questions should be answered - Not Applicable (N.A.)should be inserted where
appropriate. Legible print in black ink or typed does assist photocopying.
Your application form should by accompanied by the relevant documentation (list overleaf)
and when fully completed posted to :

Please do not forward original documents with your application - copies only are required at
the time of making the application.
Your application form should be accompanied by the following documents where relevant:
1. Copy of Long Form Civil Birth Certificate for applicant (including certified
translation if not in English or Irish)
2. Copy of Passports covering appropriate residence period (all pages please)
3. Copy of Garda Registration Certificate/Green Book (all pages) or Card
4. Documentary proof of legal residence in state (copies of immigration sta mps in
passport, official letters confirming immigration status, )
5. Copy of Long Form Civil marriage certificate (if applicable) showing civil status i.e. married, single, divorced at the date of marriage
6. Copy of Divorce papers (if applicable) - original petition and final decree in respect of
all divorces
7. Copy of Bank Statements for previous 3 months
8. Pay slips for previous 3 months if employed.
9. Copy of Documentation from Revenue Commissioners regarding tax affairs if
employed.
10. Copy of Work Permit (if appropriate)
11. Copy of E.U. Residence Permit (if appropriate)
12. Copy of Irish Medical Registration letter (for Doctors only)
13. Copy of letter confirming Refugee status (if appropriate).
For Spouses of Irish citizens who are applying after 30 November 2002 under section 15A of
Irish Nationality and Citizenship Act (INCA)1956 (inserted by section 5 of INCA 2001) the
following documents are also required:
14. Copy of Documentary proof of Irish Spouse’s entitlement to Irish citizenship (Copy of
Naturalisation Certificate / Birth Certificate if born in Ireland / Foreign Birth
Register Entry / Spouse’s parent’s birth certificate if latter born in Ireland ).
15. Sworn Affidavit made by the Irish Spouse in the presence of a legal witness
(Commissioner for Oaths, Notary Public or practising solicitor), to the effect that the
couple are living together as husband and wife and that their marriage is subsis ting at
the time of application. This Affidavit is filed with your application Form.
16. Copy of 3 proofs of residence (e.g. Bank statements, mortgage agreement, houshold
bill, rent book, social welfare books, Income tax statements etc) for both applicant
and spouse showing current address and dated within 3 months prior to date of
application.
Question 1. Personal Details

Please note name at birth is that on your Birth Certificate. Please give date of birth
in the order day, month, year.

You will be required to submit documentary evidence to support any change of name
(marriage certificate, adoption papers, deed poll etc.)

Please indicate all nationalities where dual or multiple nationalities are held.
1.3
It is your responsibility to inform the citizenship section in writing of any changes in
address. Failure to do this could result in correspondence not reaching you and
delays taking place in processing your application.

It is imperative that you detail all locations where you have resided, clearly
indicating the dates and the duration at each location. Please use extra paper if
required for this section. You will be required to account for any gaps in time periods
listed.

Please note that it is a statutory requirement for naturalisation that you intend in good
faith to continue to reside in the State after naturalisation.
Question 2. Marital Status

Please indicate your marital status at the time of application. It is your responsibility
to inform in writing the citizenship section of any changes in your marital status
during the period after receipt of your application to final decision stage.

Copy of your long form civil marriage certificate (and translation if not in English)
should accompany the application form.
2.2
For Spouses of Irish citizens
If you are applying for naturalisation under section 15A of the Irish Nationality and
Citizenship Act 1956 (as inserted by section 5 of Irish Nationality and Citizenship
Act 2001) based on your marriage to an Irish citizen you must complete section 2.2 as
well as section 2.1. You should attach copy of your long form civil marriage
certificate which should state your marital status at the time of marriage. If your
civil marriage certificate does not state this you and your spouse will be required to
swear an affidavit giving the following details:
A) Name and date of birth
B) Name of mother and father
C) Date and place of marriage
D) Name of spouse
E) Marital status (single, divorced, widowed) at the time of marriage.
Note: Your application form for naturalisation must be accompanied by the
affidavit (attached to back of application form) sworn by your Irish spouse stating
that you are living together and that your marriage is subsisting at the time of
application. This Affidavit is retained on file with your application.

This question refers to all applications for naturalisation made previously by you
both in Ireland and in any other country. List clearly the date of application, the
country in which it was made and the outcome i.e. approved or refused. For any
previous application for naturalisation in Ireland please quote the relevant reference
number 68/......, if known.

Statutory Declaration
You should sign the declaration only when in the presence of the witness. The
witness should either know you personally or should know a third party person who
