Colorado Employer Designated Provider List for
Workers’ Compensation (part 1)
This document is intended to aid you in determining
the number of designated providers that must be
supplied to an injured worker following a reported
work injury effective as of April 15, 2015.
Within 30 miles of the place of business, if there are:
If there are not two (2) physicians or corporate medical providers at distinct locations without common ownership, then you may designate physicians at
the same location or with common ownership.*
No more than 1-3 physicians or corporate
medical providers willing to treat injured employees?
If so, you may designate one (1) physician or corporate medical provider.
C.R.S. §8-43-404(5)(a)(I)(B)
No more than 4-8 physicians or corporate medical
providers willing to treat injured employees?
If so, you may designate two (2) physicians
or corporate medical providers or a|
combination of physicians or corporate medical providers at distinct locations and
without common ownership.
If there are not two (2) physicians or
medical providers at distinct locations, then you can designate two physicians at the same location. *

9 or more physicians or corporate medical
providers willing to treat injured employees?
If so, you must designate at least 4 physicians, or 4 corporate medical providers, or at least
two (2) physicians and two (2) corporate medical providers or a combination thereof.
At least one of the four (4) designated physicians or corporate medical providers must be at a distinct location without common ownership.

Under the Workers’ Compensation Act, a
physician or corporate medical provider is
deemed willing to treat an injured worker unless
they have indicated to the employer or insurer
that they are not willing to treat. C.R.S. § 8-43404(5)(a)(I)(B). However, we recommend that
you confirm the willingness to treat injured
workers with each designated medical provider
you list.

What This Means to Counties
In June of 2014 House Bill 14-1383 was signed.
This Workers’ Compensation amendment increased
the number of designated providers that an employer
must provide to the injured employee to four. There
are exceptions based on availability of physicians
in some locales. We request that you edit your
designated provider lists. Please also provide CTSI
with a copy of your revised list.
* If physicians or corporate medical providers
at the same location or with common ownership
are listed, those providers, within 5 days of the
written request by an interested party to the
workers’ compensation claim, must provide a list
of ownership interests/employment relationships
to the interested party.
