GENERAL TERMS & CONDITIONS
This agreement sets forth the terms and conditions under which Boscov’s Travel & Apple Vacations in return for payment of the amount indicated as the total price, agree to
provide this tour.
PAYMENT INFORMATION: A deposit of $150 per person is due, along with this signed reservation form in order to secure land and air space. Final payment, full names, date of
birth, and gender of all persons traveling are due on or before FEBRUARY 25, 2015. If final payment is NOT received by deadline date, reservation will be automatically
cancelled and deposit will be forfeited. No extensions or exceptions will be permitted. IMPORTANT INFORMATION: All rates and space are subject to availability at time
of booking. All rates are based on a minimum of 20 fully paid adult passengers and 10 rooms at double occupancy and are subject to increase should this minimum not be met.
AIR TRANSPORTATION: Roundtrip air transportation to Punta Cana includes air taxes, government fees and current fuel surcharges – which are subject to change/increase at
any time at the discretion of the airline. All increases would be the responsibility of the tour participant and must be paid in full prior to departure. Reservations paid in full at time of
increase/change would not be exempted. Failure to pay these charges would result in being denied boarding/travel. Once issued, airline tickets cannot be changed for rerouting.
Name changes due to spelling corrections are subject to airline rules, and penalties for re-ticketing will apply. Air penalties are determined by the airline at their discretion. Once
issued, tickets are non-refundable. MOTORCOACH TRANSPORTATION: Roundtrip motorcoach transportation to Philadelphia International Airport includes driver gratuities
and is based on a minimum of 30 full paying passengers. Should the numbers be less, the cost is subject to increase. CANCELLATIONS: Regardless of reason, cancellations
are a costly process. To offset these expenses, an administrative fee of $25.00 per person will apply. IN ADDITION, the following assessments will be incurred: from 120 to 61
days prior to arrival – $50 per person will be assessed in additional to any non-recoverable costs, including air; from 60 to 45 days prior to date of arrival - $100 per person will be
assessed in addition to any non-recoverable costs, including air; from 44 days to 31 days prior to date of arrival - $350 per person will be assessed in addition to any nonrecoverable costs, including air; 30 days or less prior to arrival or “No Shows” – NO REFUND. WORLDWIDE TRIP PROTECTOR PLAN: Available and recommended through

assumed that each individual will use all portions of the tour; there is no refund for unused portions of the tour. RESPONSIBILITIES: Boscov’s Travel, Inc. acts solely in the
capacity of agent on behalf of its patrons, arranging transportation, lodging, sightseeing, and other services, and, as such is not responsible for damage, loss, delay, injury, or
accident due to any act or default on the part of any company or person engaged in providing transportation, lodging, sightseeing, or other services which are part of this tour.
Rates quoted are based on tariffs current at time of printing and are subject to changes. CONDITIONS: Boscov’s Travel expressly reserves the right to withdraw any tour or make
