NOTES FOR GUIDANCE IN USE OF THIS FORM
A. PURPOSE OF THIS FORM – Form is used to vary the time a teacher works in one location/site.
1. This form is used to increase or decrease the fraction of time or hours a teacher works within a site during a specific time period. The minimum time must be recorded across a week, from Monday to Sunday.
2. This form is also used for permanent teachers filling emergency relief teaching positions for less than 20 consecutive working days in the current location in which they are working, another school location or a preschool.
3. This form is also used for permanent and temporary (tenured and contract) teachers changing their FTE for 20 or more consecutive working days in the current location in which they are working or another location within the same sector (ie schooling or preschooling).
Permanent teachers on leave doing emergency relief teaching need to complete a VL705 form.
This form should not be used for teachers who wish to increase their time by 0.4 or more per week for more than 20 working days.
Increases in time 0.4 or greater for longer than 20 working days should be submitted through VSP.
B. How to Complete this Form
Section 1 – Employee and Location Details
Provides all the details of the applicant necessary for identification of the location at which the increase or decrease in time is going
to occur.
Section 2 – Details of Current and New Hours / Fraction of Time
Tick the appropriate box to indicate if the change in time is Permanent, Temporary or for Emergency relief Teaching (ERT)
Specify the dates of the change – if permanent, the end date must be left blank. This will override any previous permanent hours.
Employees who are applying for a temporary change will automatically revert to their permanent hours after the specified end date.
If the form is being used for an increase of one day indicate this as a FTE over one week, Monday to Sunday.
A new roster must be completed for all part-time Preschool Teachers. The roster must detail only the times applicable to this location
and position detailed in Section 1. This is because those employees with multiple appointments must have a work roster for every
appointment.
Preschooling sector teachers must specify the new hours in decimals.
Schooling sector teachers must specify the new hours in terms of a fraction of time. Increases of greater than 0.4 must be processed
through VSP.
Account code(s) may be provided where applicable. Where no account code is provided, the salary will default to the standard
account code for the position specified in Section 1. Where salary is distributed across more than three different account codes,
please attach the details to the form.
