BEND PREMIER CUP 2016 TEAM CHECK-IN PROCEDURES Team Check In: Thursday, August 11th, 2016 from 5:00-8:00pm Mt Bachelor Village Resort, 19717 Mt Bachelor Village Dr, Bend, OR, 97701. § Check-in is for out of town teams arriving Thursday. Only one Team Representative needs to check in the team. Check in is in the Summer Twilight room in the Conference Center of the Mt. Bachelor Village Resort. (Look for the Bend Premier Cup signs). Team Check In: Friday, August 12th (for teams arriving directly to fields) or Saturday August 13th (U-9 and U-10 teams only) 2 hours prior to your first game @ Tournament Headquarters: Bend Pine Nursery 3790 NE Purcell Blvd, Bend OR 97701 § Friday, August 12th: Check-in at the Bend Pine Nursery (3790 NE Purcell Blvd, Bend, OR, 97701) at least 2 hours prior to the start of your first game. Please plan appropriately, as there is no team check in at other locations. (U-9 and U-10 teams that do not play until Saturday can check in on Saturday 2 hours prior to their first game at Bend Pine Nursery) Check-In Requirements: 1. Player Cards The Bend Premier Cup is sanctioned by US Youth Soccer and US Club Soccer and accepts pass cards from all USSF Affiliates. All players on a team must be carded under the same organization (i.e. everyone must have US Club Soccer cards or USYSA cards). • • • • Every player must have a laminated passcard with a picture on it. Region IV teams using USYSA Cards. Present your 2015-16 or new 2016-2017 USYSA State Association cards (ie OYSA, WYS). You DO NOT need travel papers, however you do need to notify your state association that you are attending the Bend Premier Cup. Teams using US Club Soccer Cards. Present your NEW 2016-2017 US Club Soccer Passcards No Travel Papers Required for Guest Players. Teams must bring appropriate player loan form from USYS or US Club Soccer, and card must be the same as rest of the team (all US Club or all USYS). Guest Players may be hand written on official rosters. 2. Medical Releases: You should have a medical release form for every player. They do NOT have to be notarized. 3. Rosters: All teams must provide seven (7) copies of their official USYS or US Club Soccer Roster at check in. This roster must include all participating players, and guest players can be written by hand so long as you have a player card for them. We will stamp the rosters and return six to you for your records. For games, you will hand a copy of your stamped roster to the referee prior to each game.
