Introducing Catalyst: A progressive solution

Introducing Catalyst: A progressive solution helping businesses protect their employees and
their bottom line.
Catalyst, a medical product from UnitedHealthcare, is now available for quoting in the New Jersey market.
Catalyst is available to new and renewing UnitedHealthcare groups in New Jersey with 51 or more employees.
Catalyst provides health care coverage by combining a comprehensive catastrophic coverage plan with a Predeductible Allowance. The value-added Pre-deductible Allowance provides consumers the opportunity to obtain
basic preventive, routine, or emergency health services without triggering the high deductible catastrophic
component of their coverage plan.
In other words, consumers can get the routine coverage they want and the protection for major health events
they may need. Some medical plans require members to meet their deductible before the plan will pay any
benefits, but with Catalyst, many of the initial medical services received will be covered under the Predeductible allowance.
Target Customers:
 Employers who are trying to manage health care costs while consumers are looking for attractive plan
options.

Employers who are looking for an alternative to HRA and provide up front coverage without the
administration of a separate bank account.

Employers who want to offer benefits and keep premium costs in check for their employees.
How Catalyst Works:
The Pre-deductible allowance is the amount the medical plan will pay, per member, before the deductible kicks
in. It’s important to note this is per member, not per family. Also, depending on the plan design, members will
have some cost sharing during the pre-deductible allowance period, such as copayments or coinsurance.
Coinsurance amounts apply toward the overall out of pocket maximum, but not the plan deductible.
It is important to know that the pre-deductible allowance is flexible. UnitedHealthcare Internal research has
shown that only 28% of individuals spend more than $1,000 per year in medical expenses, with 72% of
individuals spending less than $1,000 in medical expenses per year. With the $1,000 Pre-deductible
Allowance, necessary care will be covered for many people without triggering the plan deductible. This is how
Catalyst can provide so much to both your customer and their employees.

What is CatalystSM?
Catalyst’s Combined Services Plan
Often, medical plans require you and your
employees to meet a deductible before the health
plan will pay any benefits. Catalyst is different.
With this plan, your employees receive a predeductible allowance that can be used for not
only routine and emergency services but also
services such as inpatient and outpatient surgery,
chiropractic, maternity services and more.
With Catalyst, members have many routine
and emergency health costs paid with a “predeductible allowance.” That means the costs don’t
trigger the 100% employee-paid portion of the
plan until the allowance is used up.
Outpatient and inpatient procedures. Physician
office visits and associated lab or X-ray. Urgent
care for those weekend earaches. Emergency
care. An extensive list of medical services are
covered. Employees and their families can make
appointments for all types of care and address
health issues and are only required to pay a
copayment or coinsurance.
Beyond the pre-deductible allowance, Catalyst
also offers protection against the significant
financial losses that can be caused by major
health events.
Benefiting from a national network
Catalyst gives your employees access to
UnitedHealthcare’s national network of health
care providers. Because we offer discounted
rates for services received within our network,
an employee could have multiple physician visits
before exhausting their pre-deductible allowance.

Each family member has a pre-deductible allowance.
Preventive care will be paid at 100%
and will not reduce the pre-deductible
allowance benefit.
Does this sound familiar?1
You want to avoid the separate administration
and additional funding requirements of health
reimbursement and health savings accounts.
You haven’t provided health care coverage before for
part-time and lower-wage employees. You want to
offer meaningful coverage at a lower price point.
You want to offer a plan with higher deductibles, but
want to provide more flexible coverage than a typical
high-deductible plan design offers.
You currently offer a health reimbursement account but
find that employees are depleting those accounts and
then leaving the company.
If one or more of these situations describes your
needs, consider Catalyst for your solution.
How Catalyst works
Is this Catalyst plan right for you?
• Jake is running a high fever. After a call to
NurseLineSM, he is advised to see his doctor
for treatment.
The Combined Services Plan is:
• Jake pays a copayment or coinsurance, depending
on the type of service he receives.
• Designed to give employees the maximum
amount of flexibility when using their predeductible allowance
• UnitedHealthcare pays the remaining costs, up to
the amount of the pre-deductible allowance.
• Ideal for professional services organizations that
want to provide benefits to part-time or lowerwage workers
If, during the course of the year, Jake uses up his
pre-deductible allowance on other types of medical
care, then:
• Appropriate for employers whose employees are
knowledgeable about how to make the best use of
their health care benefits
• Jake pays 100% of the eligible expenses until the
amount of the deductible is reached.
• W hen the deductible is satisfied, Jake pays
coinsurance for the service.
$1,000 recommended
pre-deductible allowance
• After Jake reaches the out-of-pocket maximum,
UnitedHealthcare pays for 100% of covered
services received within our network.
The suggested amount for a pre-deductible
allowance is $1,000. Our research shows
that 72% of individuals spend less than
$1,000 on medical expenses annually.
Jake’s pharmacy benefit is separate from Catalyst
and does not count against the pre-deductible
allowance.

Catalyst’s Separate Routine/
Emergency Services Plan
With this plan, your employees receive separate
pre-deductible allowances for:
• Routine office visits
• Emergency care
There are two allowances: one for routine care and
another for emergency care. Each family member
has both pre-deductible allowances.
Routine care is defined as any
physician office visit and associated
lab and X-ray. Preventive care will be
paid at 100% and will not reduce the
pre-deductible allowance benefit.
