MATH PACKAGING for ESBOCES FULL SERVICE DELIVERY PLEASE NOTE: Test booklets are scanned and converted to electronic images for computer scoring so be sure that each booklet submitted has a pre-printed student bar code label (see presentation notes for placement of labels and quality control guidelines). Please remove extraneous sheets of paper, staples, clips or post-it notes from within the test booklets. Blank pages at the back of tests booklets MUST NOT be removed from the test booklets. Every delivered test booklet must have the same number of pages! BOOKLET BOX PACKAGING - ONLY SUBMIT COMPLETED ANSWER BOOKLETS: Return only test booklet 3. Submit a test booklet ONLY if a student completed at least one constructed response item. All Math Reference Sheets for Grades 5-8 must be removed from all Booklet 3’s prior to intake. Please do not include Math Reference Sheets in your boxes; they should be kept in district and returned to Questar as per the 2017 Grades 3-8 ELA/Math Test Administrator’s Manual, pg. 47. Place outlier booklets requiring special handling in labeled envelopes at the top of the box, separated by building and grade. All outlier envelopes must have the completed "Outlier Tracking Sheet" adhered to the outside of the envelope. Booklet types requiring special handling include: Large print versions of the assessment Braille versions of the assessment Students who used a word processor to complete the assessment *word processor responses are to be taped or glued flat to the response area for each question. Students who utilized a scribe to complete the assessment (be sure to transcribe student responses in their appropriate areas within the test and staple the sheets of paper used for dictation to the back cover with a single staple). *NEW PROCEDURE: you must register your boxes/envelopes in advance. The registration labels must be attached to the box or envelope in an area where it will not be damaged when opened. See “Box Registration Instructions” for further details. Foreign language versions of the assessment should be packaged underneath the outlier envelopes and gathered together at the top of the English versions of the assessment. Foreign Language assessments should not be placed in envelopes. Multiple grades may be packaged in the same box as long as the grades are bundled separately. Envelopes may be used for grades/districts with low enrollment. Use sturdy boxes no larger than standard paper boxes. Smaller boxes may be used to prevent small amounts of tests or answer sheets from shifting within the box. ANSWER SHEET PACKAGING - MUST BE PACKAGED SEPARATE FROM BOOKLETS Answer sheets MUST NOT be packaged along with the test booklets. Answer sheets gets submitted to Student Data Services directly upon your arrival at intake. You must include all completed rosters in the ANSWER SHEET box.
