1. Specifications of operation to be filled out by the GEUS project leader responsible for a Full Charter
Operation. These specifications must reach Company Name no later than 1 March for the year in question.
2. Order confirmation to be filled out and signed by Company Name specifying equipment and flight crew
for the helicopter services required. Order confirmation must be sent to the assigned project leader at GEUS

3. Signature page to be signed by both parties to confirm the order specifications.
