Senior Jersey Order Form
Below are pictures of the Senior Jerseys.
The front of the jersey will be the same for all (left). The back of the jersey will be a
customized name (right). You may print your first name, last name, nick name, or any other
APPROPRIATE saying. Parents are also invited to purchase a jersey. It is tradition that seniors
and their parents wear the senior jersey on the first day of school for the senior breakfast and
big entrance into the school.
Jerseys sizes small, medium, large, and XL cost $35; sizes XXL and XXXL cost $40. Checks
should be made payable to PHS Class of 2017. Orders will be taken through May 20, and
payment is due with order. Order form and payment should be turned in to Mrs. Romines in
Room C220. Jerseys will be ready for pick up in the Commons at Verification of Residency in
July.
