on this form at the time of signing are true and complete. I further declare that the Auction Lot Numbers are correctly matched with Official
Ear Tag Numbers and that passports relating to these lot numbers are correctly matched or I authorise the auctioneers to act on my behalf
without any responsibility attached to this action in respect of ear numbers and passports. I hereby declare that no animals have moved onto the
above holding within the last 6 days. I confirm that I have identified any imported animals which must be declared by the auctioneer at the
point of sale.
TB LEGISLATION- All cattle over the age of 42 days (with effect from 01.03.07) within a 1+2yr testing parish must be accompanied
by a copy of a negative TB test certification within 60 days prior to movement. Vendors presenting cattle in the market that have not
had a clear TB test are liable to prosecution. Bentham Auction Mart will take no responsibility for ineligible cattle entering the market
and the vendor indemnifies the auction mart and its staff from any consequential loss claims.
WARNING: The making of a false declaration or the supply of an animal which is accompanied by a false declaration may constitute an
offense under section 1 of the trade descriptions act 1968, for which the maximum penalty on summary conviction is a fine not exceeding
£5000 and a conviction on indictment, a fine or imprisonment for a term not exceeding two years or both.

 Withdrawal periods have been observed for all veterinary medicines and other
treatments administered to the animals while on this holding and previous
holdings.
 To the best of my knowledge the animals are not showing signs of any disease or
condition that may affect the safety of meat derived from them.
 No analysis of samples taken from animals on the holding or other sample
has shown that the animals in this consignment may have been exposed to any
disease or condition that may affect the safety of meat or to substances likely to
result in residues in meat.
