Cassidy Dr. Wausau, WI 54401 [email protected] 715-212-7339 Policies and Procedures for Tutoring in Wausau Invoice and Payment Policy A monthly statement of tuition charges for the upcoming month will be given to the student by the first tutoring session of the month. Additional charges for workbooks, other materials, or travel expenses will be included if applicable. Payment (check, cash, or via paypal) is due by the 15th of the month unless other arrangements have been made. Tutoring will cease if payments are not paid in a timely manner and kept current. Attendance Policy Your student’s sessions are standing appointments. This special time is a commitment on your part and on mine. Consistency in tutoring is a primary factor in achieving educational growth. If your student is ill, or otherwise unable to attend the tutoring session, please call me in advance to notify me of the emergency. It is the responsibility of the individual(s) who have signed this contract to reschedule the missed lesson. If Darlene Larson is unable to attend the session, it will be her responsibility to reschedule the lesson. Sessions that are scheduled will be billed unless there is mutual agreement ahead of time to make a change in the schedule. There are no refunds for missed or last minute cancelled appointments. If a make-up session cannot be arranged, the tuition fee for the missed session will be considered earned by Darlene Larson. Text messages are generally the best way to reach Darlene if an immediate response is required. Safety and Learning Environment Prompt arrivals and pick-ups are essential. Appointments are often tightly scheduled, and while there is a waiting area, there is no supervision available. I urge parents to stay with children if there is going to be a wait time. Your student’s safety and security is critical. Individual(s) identified in this contract who have retained services are invited to attend the tutoring sessions in an “in the background” listening mode unless we have made other arrangements about the parent’s role during the tutoring session. Please avoid bringing siblings of the student or other children to the tutoring session because this may distract the student being tutored. Contract Renewal A Tutoring Contract, outlining the tutoring dates, times, rate, and length of contract (usually 3-9 months) will be generated when tutoring begins and at renewal dates. Parents are asked to confirm receipt of and acceptance of the Tutoring Contract and The Reading Spelling Connection’s Policies and Procedure via email. Confidentiality Policy Darlene Larson acknowledges that, during the course of performing the services described in this contract, confidential information will be developed specific to the student. Darlene Larson agrees not to disclose, directly or indirectly, the confidential information to any third person or entity, other than the individual(s) identified in this contract who have retained her services unless requested to do so by them. Darlene Larson may disclose any confidential information that is required to be disclosed by law, government regulation or court order. Consultation and Reporting Policy I am available to the individual(s) identified in this contract who have retained services of a student for a brief professional consultation regarding the student at no additional charge. Please let me know at the beginning of a session that you would like to meet with me, so I can end the tutoring session early enough to consult with you. At times, I may request a short meeting with you as well, to update you on your student’s progress. If more time is needed, I would be happy to set up an appointment with you. For ongoing students, I can also test and prepare a yearly written progress report if desired. A meeting to discuss the report would be scheduled at no additional charge. Other Services Because I specialize in tutoring students with reading difficulties, there are certain things that are not included in the scope of this agreement. in-depth testing advocacy work; i.e. attending IEP or 504 meetings. assisting with homework. consulting with or contacting the student's teachers or other extended relatives If in-depth testing, consulting services, etc. are desired, please contact me separately to determine whether I am able to help you or can offer a referral to someone who can. Travel Expense Policy Travel mileage expenses, if applicable, will be calculated to and from my home (7657 W. Cassidy Drive, Wausau, WI) or my office (1500 Merrill Ave., Wausau, WI) to the alternate tutoring or meeting location at the current IRS mileage rate.
