Member Benefits
Based on your member plan, you may receive
the following services:
Health education and counseling
How do I become a member?
Medically necessary radiology and lab services
Those who qualify must meet certain age, financial,

residency and health criteria. In addition, you are

practitioner to provide treatment for illnesses
or chronic disease

What does it cost?
Immunizations such as flu and pneumonia shots
Co-pays are required for services and are based
upon a sliding scale according to your income,
family size, and ability to pay. Co-payments are
due at the time you receive your services.
Member Services
You will be able to get medical services that
are medically necessary and normally provided
at The Medical Center of Central Georgia.
You will be assigned a clinic site for your health
care home.
Assistance with prescriptions if you have no
other prescription plan
Your Responsibilities
In order to remain a member of the program, you
must demonstrate your partnership by:
Keeping your appointments
Following your plan of care prescribed by your
doctor or nurse practitioner
Following the Code of Conduct
