NOTES:
 All tubes with additives must be gently mixed 6 – 8 times
 Blue tubes must be filled to the minimum fill line (groove in tube) – others may be half full unless specifically required to be full
 Fill tubes from syringes immediately after draw (gently mix)
 Blood Bank Tubes MUST have BB# and 1st initial-full last name of phlebotomist if for possible transfusion (All Inpatients)
 Blood Cultures require a 30 second Chloraprep scrub – Alcohol for infants less than 2 months of age
 Tourniquet should not be left on longer than 2 minutes (closer to 1 minute is preferable)

Note regarding ETOH: May add ETOH onto unopened SST or Green. Enter Changed Specimen Type in LIS. Enter Comment: add-on ETOH/ not collected per ETOH protocol.
