TENNIS AFTERSCHOOL
REGISTRATION FORM
Since 2001, Coach Larsen has provided Tennis
Afterschool lessons with a clean safety
record! Lessons are conducted in a non-competitive
environment. Lessons encourage “goal-setting”, build
self-esteem, basic tennis skills through fun games,
plus offers opportunities to be a team captain or
organize "round-robin" court play. Beginners start on
the ball wall. Intermediates practice consistency and
stroke production lessons on the basketball court.
Racquets balls, portable nets are provided. Enroll
using this Student Registration & Waiver forms.

1. "Meet & Greet" @ ball wall. If rain, meet in
Kindergarten room.
2. Pick up/Fill-out Registration and Waiver
Forms.
3. Cost: $120, payable to TennisAfterschool
(Funds are not refundable. Lessons can be redeemed
in the summer)
---If @ 2/28, Parent's Meeting: Fee can split into 2 payments, 1 due
at registration, other at end of March. (Checks, cash
accepted/receipt given)
---If unable to attend 2/28 Parent Meeting, Deliver

risks in connection with the student’s
participation in any and all of the PTA
sponsored activities. I (we) hereby release and
discharge the Tennis Afterschool, Lei Larsen,
California State PTA, all PTA officers, employees
and agents from all liability, claims or demands
for any damage, loss or injury to the student, the
student’s property, or parent’s property in
connection with participation in these activities.
I do hereby certify that to the best of my (our)
knowledge and belief that my child is in good
health. In case of illness or accident, permission
is granted for emergency treatment to be
administered. It is further understood and
agreed that the undersigned will assume full
responsibility for any such action, including
payment of costs. I confirm that I have read the
afterschool enrichment information in the ASE
packet provided by our school/or at
[email protected]. I understand that
changes and late admissions are at the
discretion of the class vendor. I also understand
that my child cannot be left unattended before
or after class per our school principal and the
Walnut Creek Unified School District Policy and
that disruptive behavior is grounds for removal
from ASE programs without a refund. I (we)

B. Coach brings racquets, balls, nets, etc.
Students who bring their racquets to school,
must keep racquet inside their back packs, until Coach gives permission to
remove for lessons.
C. ATTENDANCE is taken daily. If your
student will be ABSENT, PLEASE ADVISE
Coach Larsen in writing a day in advance.

D. RAIN-DAY: Coach determines and
advises if class can be conducted indoors to
review tennis academic components.
----If a class is cancelled, during 2017
Spring session, Coach or student/families
may "save the date" to receive a 1 hour
tennis lesson during the summer
program Tennis Afterschool program.
Spring 2017 Tennis Afterschool ATTENDANCE Card: Coach
will "punch hole" in card to reflect attendance by
student. Card reads 10 lessons.
Example: If only 9 holes in card, then student can bring to
use for a 1-hour free lesson during Summer, 2017 tennis
lesson.
