POSITION DESCRIPTION Arrowhead Regional Development Commission Job Title: Classification: Division: Reports to: FLSA Status: Evidence-Based Program Coordinator Program Coordinator Area Agency on Aging Director of Area Agency on Aging Non-exempt Job Summary: Develops, coordinates and supports evidence- based health promotion and disease management programs in the Arrowhead region. Includes; relationship development, program implementation and coordination with partners to schedule classes, report outcomes, track and support projects, database management, maintain inventory of training supplies, schedule and promote classes and leader trainings, and communicate with existing and new partners to support program sustainability and expansion. Key Functions and Responsibilities: pg. 1 Develop and implement evidence based programs for the Arrowhead Area Agency on Aging Build and maintain effective relationships with partnering organizations, including aging service providers, community based providers, health care providers, faith communities, counties and others in the region to support delivery of evidence based programs. Coordinate scheduling and provide direction to program volunteers and/or student interns. Fulfill Master Trainer requirements as feasible for evidence based programs offered regionally to facilitate local training for training leaders, coaches and facilitators. Provide monitoring and oversight of evidence based program delivery, reporting and fidelity to maintain quality control and integrity of programs. Research and evaluate new evidence based programs to meet identified needs in the area and develop local capacity as available resources allow. Give public presentations and provide outreach for evidence based programs. Work in partnership with statewide efforts to promote the MN Department of Human Services vision for evidence-based programs. Use state-approved technology and tracking systems to document program participation. Submit all required reports in a timely manner Participate in EDP Team, Aging Team and Area Agency on Aging Division meetings and other agency work groups. 8/15/2016 Work with team members and provide support in projects, initiatives that focus on building capacity for evidence based programs in the Arrowhead Region. Maintain accurate list of evidence-based health programs (EBHP) leaders and partners Data entry to meet state, federal, NCOA and licensing agency requirements Communicate with leaders and partners through newsletters, electronic communication and mailings to foster and maintain relationships with partners and volunteers. Participate in agency and division functions and staff events as needed or requested and other functions as assigned to promote aging services in the arrowhead region. Education/Experience: Bachelor’s Degree (B.A.) with minimum of one-year experience in health and human services field related to aging or health promotion, or Associates Degree (A.A.) or equivalent from two-year college with 5+ years’ experience in a related field of work. Competencies: Demonstrated program development experience and volunteer recruitment/management experience. Ability to establish and maintain effective working relationship with project partners, including health care partners and faith based organizations Strong organizational skills and careful attention to detail with the ability to establish priorities Ability to develop and manage program budgets. Mathematical Skills: Ability to work with mathematical concepts such as probability and statistical inference, and fundamentals of plane and solid geometry and trigonometry. Ability to apply concepts such as fractions, percentages, ratios and proportions to practical solutions. Reasoning Ability: Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with concrete and abstract variables. Physical Demands: The employee is regularly required to talk or hear. The employee is frequently required to stand, walk, sit, use hands to finger, handle or feel, and reach with hands and arms. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required include close vision, distance, color vision, peripheral vision, depth perception and ability to adjust focus. Work Environment: Employee works in a professional office. The noise level in the work environment is usually quiet. There are no hazards or unsafe conditions in this environment. Travel: Valid driver’s license is required. Position requires moderate travel within the region, occasional travel out of state, attendance at night meetings and minimal overnight travel.
