TRIAD
Green Lake County TRIAD focuses on any issue
relevant to Senior Citizens. These issues focus on
Crime Prevention and Awareness, Safety-related
topics, and virtually any quality of life issue which
impact our seniors. The Berlin TRIAD was formed
in 2002 and is closely affiliated with the Green
Lake County TRIAD. Meetings held: second
Tuesday of each month: 8:30 a.m. in the Green
Lake County Sheriff’s Office or at 12:30 p.m. at the
Berlin Senior Center.

Eligibility is based on a significant delay in one or more areas of

Families are eligible for services if they have a child with a severe
disability, under the age of 21, and living at home. A severe
disability is a physical, mental or emotional limitation which
seriously restricts the child’s ability to carry out basic daily living
activities such as self-care, learning, communication, mobility,
and self-direction.

Although family living income is not a basis for eligibility, costsharing may be required on a sliding fee scale.
The Katie Beckett Program is designed for children 0-18 with
chronic long-term disabilities or complex medical needs living at
home. The child must require a level of care provided by a
hospital, skilled nursing home, or intermediate care facility.
Family income is not a factor. If eligible, the child would receive
a Wisconsin Medical Assistance card, which could cover a
number of prescribed services.
Family Support is a state-funded program. Because each county
has limited funding for this program, eligibility does not guarantee
a family will receive services. Agencies may have waiting lists for
services and assessments. Administering agencies may make
the final decision on who is served and approve each family’s
service plan. The contact person at the county is the Family
Support Coordinator.

Head Start provides comprehensive health, nutritional,
educational, and social services to primarily low-income young
children and their families. Head Start must serve children with
disabilities. At least 10% of their state-wide enrollment must be
children with disabilities.
The guidance of the Individual with Disabilities Act suggests that
preschool age children with disabilities may receive their special
education and related services in non-public school settings such
as Head Start or other child care programs as long as the special
education and related services meet the child's IEP and are
under the supervision of the public schools.

Under the Individuals with Disabilities Act, it is the sole legal
responsibility of the public school to provide special education
and related services to eligible children with disabilities from age
three to twenty-one. The IDEA regulations state that children
with disabilities should be educated alongside their typically
developing peers unless their disability prevents them from
succeeding in the regular education environment with
supplementary aides and services.

Supplemental Security Income (SSI) is a monthly cash benefit
paid by the federal Social Security Administration (SSA) and
state Department of Health and Family Services (DHFS) to
people in financial need who are 65 or older or people of any age
who are blind or disabled and residents of Wisconsin.
You may be eligible for SSI payments if you have little or no
income or if your assets are $2,000 or less for a single person or
$3,000 or less for a married couple. Certain assets do not count,
such as your home or car.

Federal Consumer Information Center helps other Federal
agencies and department develop, promote and distribute
consumer information to the public. The quarterly Consumer
Information Catalog lists more than 200 free or low cost booklets
on such topics as health, federal benefits, money management,
housing, child care, employment, small business, food and
nutrition, consumer protection and more. Visit the website listed

updates from various federal agencies.
FCIC’s National Contact Center serves as a single point of
contact for individuals with questions about Federal agencies,
programs and services. The specially trained staff can answer
questions in English and Spanish about all aspects of the
Federal government or direct callers to an appropriate contact.
A consumer Complaint Bureau which will investigate and report
any complaint received by them concerning any type of
insurance. This office has complete control over all insurance
transactions in the state.

EDUCATION, TRAINING AND EMPLOYMENT SERVICES
The Hearing Aid Dealers and Fitting Board was created to
license and regulate hearing aid dealers and fitters because
many seniors have problems with hearing aids that do not work
well. If you have any questions about your hearing aid dealer,
contact the Examining Board to find out if he/she is licensed or
has complaints on his/her record. If you have a complaint about
a dealer, they will investigate. Some of the violations you may
contact them about are: (1) failure to properly test hearing aid of
the person before selling a hearing aid; (2) telling a person that
a hearing examination is the same as a medical examination or
recommending a treatment for a medical condition; (3) obtaining
or making a sale by fraud; (4) showing a model that is not
available to customers in order to sell them a different model; (5)
failure to supply a receipt to a customer who buys a hearing aid –
the receipt must specify the terms of any guarantee and must be
marked “used” when the hearing is not new; (6) incompetence
or negligence in fitting and selling hearing aids. This service is
free to anyone who has a hearing problem.

Agricultural Agent
Agent available to assist clients with farm-related problems and
offer agricultural education opportunities. Agent offers
educational programs in the areas of agricultural business, crop
production, dairy production, and general farm production. The
Agent also provides horticulture education.
Family Living Agent
Provides education and information to individuals and families in
the following areas: parenting, money management, nutrition
and food safety, health, and adult caregiving.
4-H and Youth Agent
4-H Youth Programs offer educational opportunities to youth
ages 5 to 19. Youth gain lifelong skills while learning about
technology, agriculture, arts, and science. Youth and adults
participate in community-based clubs, county fair, community
service, and county and state-wide events.
Federal (VA) and State (WDVA) programs are available for
veterans and their dependents and/or survivors.
WDVA
programs include home loans, home improvement loans, multipurpose loans, educational and medical grants, membership to
the Wisconsin Veterans’ Home at King, WI, and other
miscellaneous programs.
VA programs include pensions,
compensation for service-connected conditions, home loans, GI
bill, insurance, burial benefits, headstones and many
miscellaneous programs and services.
VA hospitalizations and out-patient services and medications are
provided to qualified veterans. Transportation to VA facilities is
provided to qualified veterans. A Veterans’ Service Commission
for the county provides temporary aid to eligible veterans and
their families according to state statutes. A fund is also available
to eligible veterans for indigent burials. Flag holders are
provided for all veterans' graves in the county.

The Aging & Disability Resource Center serving Adams, Green
Lake, Marquette, and Waushara counties provides information
and assistance to older and disabled adults, their friends,
family, caregivers and the general public. We can assist by

Assists with appropriate referrals to other agencies based on
customers' requests and needs. The staff is trained and

Aging Services Advisory Committee
The Green Lake County Aging Advisory Committee is the policy,
planning and community-organizing group designed by the
County Board to be the focal point for County Aging Activities.
Appointments to the Aging Advisory Committee are made by the
County Board Chairman with the approval of the Green Lake
County Board, and at least one-half of the Advisory Committee
representatives must be 60 years of age or older.
The Aging Advisory Committee makes decisions concerning
services and funding for the nutrition program, transportation and
Senior Centers. The Aging Advisory Committee invites you to
attend meetings to give your input on aging services and issues.
Alzheimer’s Family and Caregiver Support Program
This program provides funding for services needed by the
caregiver or family members of the Alzheimer’s patient. Services
could include respite for the caregiver, assistance with
housekeeping, adult day care, information and referral, special
adaptive equipment and transportation reimbursement.
A
physician’s diagnosis of Alzheimer’s Disease or one of 9 other
dementias is necessary for eligibility for this program. Funding of
services is based on income and expenses incurred by the
Alzheimer’s patient. Call Green Lake County Department of

Benefit Specialist
Provides congregate and home delivered meals, Benefit
specialists, transportation, senior centers, Elder Abuse
Investigator, Caregiver’s support, Information & Assistance and
other programs for people 60 and over.
The Benefit Specialist provides direct services and
representation to individuals (age 60+) on all state, federal and
county benefit programs and acts as an advocate on their behalf
with information and assistance. This service, provided through
county and tribal Aging Units, is supported by legal back-up
through the Area Agency on Aging with the State Department of
Health & Human Services without any charge to the senior
citizen.

Community Option Program (COP)

The Community Options Program is a State program funded by
Medical Assistance to provide services to help Green Lake
County residents remain in their own homes or in another
community living arrangement instead of a nursing home or
hospital. Services provided could include help with household
cleaning, assistance with daily living activities, companionship,
meal preparation, shopping, home modification, or special
adaptive equipment.

Disability Benefit Specialist
The Disability Benefit Specialist (DBS) helps answer questions
and solves problems related to Social Security, Medicare,
health insurance and other public and private benefits for
people with disabilities.They serve people ages 18-59 with a
physical or a developmental disability, a mental illness or a
substance abuse disorder. Services are free and confidential.
Family Caregiver Support Program
The Family Caregiver Support Program (FCSP) provides funding
and services to aid caregivers in their caregiver role. In order to
be eligible for financial assistance for services, you must either
be the grandparent or older relative caregiver age 60 or older
who has legal custody or guardianship of a child age 18 or
younger or you must be an adult family member or another
individual who is an informal or unpaid provider of in-home
and/or community care to an individual age 60 or older. This
FCSP providers assistance to caregivers in gaining access to
supportive services such as supportive home care,
transportation, respite care and/or adult day care. A support
group is held monthly and is open to all family caregivers. The
Aging/Long Term Support Unit also has a loan library filled with
resource materials such as videos and books on caregiving and
other related topics.

Supportive Home Care
An agency program to provide a variety of services to assist
senior citizens to remain in or return to their own homes from a
hospital or nursing home. It may involve finding someone to
come into your home to do daily activities that the senior citizen
is unable to do for themselves, such as house cleaning, laundry,
meal preparation, snow shoveling, lawn mowing and personal
hygiene. For income-eligible applicants, the agency can help
pay for a provider of these home services. Call Green Lake

Section 49.30 of the Wisconsin Statutes authorizes counties to
provide burial services, for persons who are receiving
Supplementary Security Income (SSI) or some categories of
Medical Assistance (MA).
There are income and asset guidelines that must be met to
qualify for the program. All WFCAP reimbursement requests for
residents will be processed centrally by the Enrollment
Management Central Application Processing Office (EM CAPO).

East Central Income Maintenance Partnership
Medical Assistance, also known as Medicaid, is a joint federal
and state program providing health care benefits to persons with
limited income and financial resources. Even if your income
exceeds the maximum eligibility levels, you may still qualify for
Medical Assistance because of the program “spend-down
provision” (incurring medical expenses greater than your excess
income for a six month period). Medical Assistance is available
to those eligible people living in their own homes or in nursing
homes. Medical Assistance usually covers those medical costs
not paid by Medicare.

Hospice Hope is a program for persons with a limited life
expectancy of six months or less who desire more aggressive
treatment. An interdisciplinary team works with patients and their
families dealing with their physical, emotional, social and spiritual
needs, at a time when a curative approach to care is no longer
appropriate.
Through skillful pain and symptom management and the
supportive services of a professional and volunteer team, every
effort is made to provide comfort and to enhance the quality of
life for patients and families, enabling them to live as fully as

the Green Lake County community by engaging in meaningful
and fulfilling volunteer service. RSVP works to address
community needs by placing volunteers age 55 and over in
area social service organizations. Some of the opportunities
available for volunteers include reading with children,
befriending the elderly, responding to emergencies, and
assisting in home construction for low-income families.

A one-time $225 payment can be made to an eligible surviving
spouse following the death of an insured male or female worker.
The Social Security Administration office in your area should be
notified of the insured worker’s death and a formal application
must be done to receive this benefit. The surviving spouse will
then receive either his/her own Social Security benefit or that of
the deceased spouse, depending on which Social Security
benefit is larger.

Medicare is a Federal health insurance program for people 65 or
older and certain disabled people. Medicare has two parts –
hospital insurance and medical insurance. Hospital insurance
can help pay for inpatient hospital care, inpatient care in the
skilled nursing facility and home health care. Medical insurance
can help pay for medically necessary doctor’s services, outpatient hospital services, and a number of other medical services
and supplies that are not covered by the hospital insurance part
of Medicare. Medical insurance also can pay for home health
care services. Men and women who are not receiving monthly
social security checks should contact a social security
administration representative three months before they turn 65 to
sign up for Medicare. Those people currently receiving social
security checks will receive their Medicare care in the mail
automatically.

Supplemental Security Income (SSI) is a Federal Program for
people 65 and older or blind/disabled individuals who have
limited income and resources. To qualify, an individual is

The ownership of a home, care and ordinary
household furnishings are not counted as assets. You must
provide proof of your income and assets. If you are eligible to
receive SSI payments, you will automatically receive Medical
Assistance Health Insurance (Medicaid). Some people qualify
for both regular Social Security benefits and SSI, some for Social
Security only, and some for SSI only.
Survivor’s Insurance
Survivor’s benefits are available to families of insured workers
who die at any age. Benefits are paid to the deceased worker’s
spouse or divorced spouse if they are married at least 10 years.
You must be at least 60 years old or 50 years old and disabled to
receive this benefit. You must provide the same information

prevocational, supported employment, and adult day
services to developmentally disabled and chronically
mentally ill adults. Supported employment services
include job assessment, job development, job placement
services, job coaching services, representative payee
services and transportation services.
Provides services to children and families including the
following: child abuse/neglect services, juvenile services,
placement, crisis intervention, day care, domestic violence
services, intensive supervision.

Clinical Services Unit
Services to adults and children. Includes mental health
counseling, AODA counseling (individual and group),
community support program (CSP) for chronically
mentally ill, crisis intervention and related services.

Economic Support Unit
Administers economic benefit programs including W-2,
Food Stamp Program, Child Care, Long Term Care
Services and Medical Assistance.

Also administers the Wisconsin Home Energy Assistance
Program (WHEAP) which is a one-time benefit payment
for each heating season (October 1 – May 15). Intended
to help you pay a portion of your heating costs. Can
either rent/own home, and can qualify even if heat is
included in rent or wood is a heat source. Public Benefits
Program has enabled the Energy Program to expand
services to assist with non-heating electric costs.
Applicants must meet eligibility guidelines, which change
each year.

Senior Citizen Mealsites/Centers
Throughout Green Lake County, Senior Citizen Centers provide
a meeting place for Senior activities. Some of the Centers’
activities include card playing, bingo, crafts, potluck luncheons
and educational speakers/classes. The service activities may
include nutrition meal sites, telephone reassurance,
transportation and more. All seniors are invited to attend the
Centers at any time.
The nutrition program for the elderly provides older persons the
opportunity for low cost, nutritious noon hot meals in group
settings. Transportation to and from mealsites is provided on
request. Donations for meals are contributed to the program
according to each participant’s ability to pay. One must be sixty
years old or older in order to qualify for this program.

The Berlin Hospital Mobile Meal Program is designed to insure
that a nutritious meal is provided to homebound Berlin area
residents who would not otherwise be able to receive a daily
balanced meal. The program is for those elderly, disabled or
convalescent Berlin area residents that are temporarily or
permanently unable to leave their homes even when
transportation is available or when a relative is unavailable to
help. All participants in the Mobile Meals Program will have their
eligibility for service screened on a quarterly basis by the Mobile
Meals Board. The Program is administered by the Berlin
Hospital Auxiliary.

Hospice Hope is a program for persons with a limited life
expectancy. An inter-disciplinary team works with patients and
their families, dealing with their physical, emotional, social and
spiritual needs, at a time when a curative approach to care is no
longer available. All families who need and desire hospice hope
will receive routine home care services by a nurse and social
worker regardless of financial resources and available insurance.

HHA services are also provided along with volunteer support to
allow persons to stay in their own homes.
Classic Cab, LTD. is a transportation program funded by city,
state and federal funds, which provides both taxi and
ambulance services.
TRANSPORTATION
All areas of Green Lake County are provided with "response to
call-in", door-to-door transportation services to any person 55
years of age and older or any handicapped person. The priority
areas of the programs are to provide transportation to medical
appointments, congregate meal sites, and other personal
appointments necessary for daily living. The services are
available five days a week, Monday through Friday. The
transportation director should be called two days before a local
ride is needed and one week before medical appointments in
larger cities. In Green Lake and Princeton, transportation is
done by volunteer drivers using private vehicles. In Berlin and
Markesan, transportation is done by a van this is equipped with a
wheelchair lift for the handicapped.

Trips are generally reserved for individuals in the Berlin City
limits, however if there are enough drivers available, services
may extend outside city limits, but are dependent on the
number of miles. If possible, it is recommended that trips be
scheduled at least one day in advance to ensure availability.

Assists towns with creating goals for their communities relative
to the way that they develop, and to regulate land uses within
the county through the enforcement of comprehensive zoning,
shoreland, subdivision, and floodplain ordinances, as well as
related Wisconsin Administrative Codes, to achieve those

permitting process whereby applicants identify anticipated uses
and structural setbacks and the department ensures
