on the back page of this application form.
If the Site Layout Plan does not conform it will be returned and
your application will not be processed until corrected.

and postal address, with the required MIC per unit. Please indicate if any unit/house/apartment will require a time of day electricity tariff.
Suitable loads for a time-of-day electricity tariff include storage heating or underfloor heating incorporating a heat pump.

Commercial units can be connected by either means. Metering arrangements will be confirmed by the local ESB Networks DAC
design office.
Please note that any subsequent changes to addresses will incur additional charges. Use more pages if required
IMPORTANT REQUIREMENTS
Your application will not be processed if any one of the following is incomplete or missing. The accuracy of the information provided will
influence the design of the network to meet your requirements. Please ensure that all numbering, addressing, and MIC information for
all individual units/houses/apartments is provided. Subsequent changes to this information will require ESB Networks DAC to issue a
revised quotation/design and additional charges will apply.
Ensure that the following is completed and returned:
• Fully completed and signed application form.
• Ordnance Survey site location map of minimum size A3.
• Copy of architect’s site layout plan, of minimum size A3 and of minimum scale 1:500 showing final name of development, addressing,
and numbering.
• Electronic copy of site layout plan as described on the last page. Email to [email protected]
• Copy of letter from Local Authority confirming the final name of the development.
SAFETY
Construction work under ESB Networks DAC (ESBN) overhead lines is not permitted and ESBN will instruct that any such work is
discontinued. Should developers fail to comply with such an instruction, the Health and Safety Authority will be requested to intervene.
ESB Networks DAC produces safety material in a variety of formats including booklets, videos, DVD’s and posters. Please visit our web
site at esbnetworks.ie for information on safety.
If ESBN lines/cables exist on site, maps of the site must be requested from ESB Central Site at Fax. 01-6388169 or by email to:
[email protected] and appropriate safe systems of work must be implemented when working in the vicinity of such overhead electricity lines
or underground cables - refer also to relevant HSA approved Codes of Practice.
DATA PROTECTION
ESB Networks DAC complies fully with the Data Protection Act. Unless you request us to do so, personal data provided by you in this
application form will not be disclosed to other parties except in the circumstances below:

ESB Networks DAC will use the information supplied on this form for the purposes of managing your electricity supply connection and
the electricity network and performing its functions as distribution system operator. In performing these functions, ESB Networks DAC
may utilise the services of contracted third party data processors. ESB Networks DAC may disclose your data to these third party data
processors, which are only permitted to use your data as instructed by ESB Networks DAC. They are also required to keep your data safe
and secure. By signing this application you consent to this use of your personal data.
•
ESB Networks DAC may make available the existence, location and/or technical aspects of your connection to licensed Electricity Supply
Companies and other parties involved in your electricity supply. In the case of new connections, ESB Networks DAC will make available
your telephone contact number to licensed Electricity Supply Companies in order to facilitate energisation of the connection. By signing
this application you consent to this use of your personal data

If you contact us requesting energisation prior to securing a supplier, you authorise ESB Networks DAC to disclose the details on this form
to the Public Electricity Supplier who will then default as your supplier. By signing this Application you consent to this use of your personal
data.

Please note that ESB Networks DAC may be required by law, or our license obligations, to provide data that ESB Networks DAC holds about
you, your electricity supply or connection, to government agencies or departments, the Commission for Energy Regulation or other third parties.

Contact details may also be provided on a strictly confidential basis to a professional third party market research company, for the sole
purpose of researching your satisfaction with the services provided by ESB Networks DAC and to help enhance our service. Please

SPECIFICATION FOR ELECTRONIC MAPPING
• The Developer must provide an acceptable copy of the Site Layout plan in an electronic CAD file format. The preferred CAD format is
Microstation DGN but Autocad DWG and DXF will also be accepted.
• The Site Layout plan must contain a roof outline plan of the development (i.e. without internal building details such as furniture, fixtures
and fittings, etc). A block outline plan is required for apartment blocks.
• The Site Layout plan must include all roadways, footpaths, kerbing, parking spaces and property boundary/divisions.
• The Site Layout plan must be free of such items as contour lines, paving, landscaping, foul sewers, storm sewers and water mains.
Cross-hatching/patterning should not be included on the drawing except where it is difficult to determine the outline of buildings.
• The Site Layout plan must be labelled with a north point and the Ordnance Survey Map reference, and must contain at least 4
reference points to the underlying Ordnance Survey map (e.g. existing street corners, benchmarks, etc)
• The Site Layout plan must be labelled with the final street names and house numbers.
• The Site Layout should be emailed to this address: [email protected]
