Security Health Plan’s HMO/POS Private plans are rated 4.5 out of 5 among NCQA’s Private Health Insurance Plan Ratings 2015-2016. 5 Specialists, referrals and out-of-network care We have more than 8,000 providers If you need to go to a specialist, including for behavioral health and in-patient hospital care, you may see any provider listed in the Provider Directory without a referral. It is a good idea to discuss your care with your primary care provider first so that it can be better coordinated. If a provider suggests you receive care from someone other than your regular provider, you will need to find out whether the provider is in Security Health Plan’s network. You might need to receive prior authorization for certain services. Some services may also require additional member education on treatment options, which we offer as part of our shared decision making program. A list of services that require prior authorization can be found at: www.securityhealth.org/authorization You can find an affiliated provider in any of the following ways: • Review your Security Health Plan Provider Directory. • Check our website at www.securityhealth.org/etfnetwork. • Contact us at 1-800-472-2363. 6 Your out-of-area students are covered If a dependent is a full-time student attending school outside of the Security Health Plan service area, the following services are covered: • Urgent or emergency care. Non-urgent follow-up care out of the service area must prior authorized by Security Health Plan or it will be covered. • Outpatient mental health services and treatment of alcohol or drug abuse if the dependent is a full-time student attending school in Wisconsin, but outside of the plan service area. In this case, the dependent may have a clinical assessment by a non-plan provider that Security Health Plan designates. If outpatient services are recommended, coverage will be provided for five (5) visits outside the plan’s service area. Additional visit must be approved by Security Health Plan. If the student is unable to maintain full-time student status, he/she must return to the plan’s service area for treatment to be covered. Eligible child dependents who are full-time students at an accredited vocational school, accredited technical school, accredited adult education school, accredited college or accredited university living outside the service area for the purpose of school may obtain follow-up care from non-affiliated providers near their school. This does not include students in secondary schools. Coverage for follow-up services provided by non-affiliated providers is limited to services received within 120 days after receipt of urgent or emergency care, or within 120 days after medical service for an injury, surgery or acute illness provided by an affiliated provider or a Security Health Plan approved referral provider. Routine care, such as an annual physical exam or contraceptive care is not considered follow-up care. You must contact our Customer Service Department at 1-800-472-2363 to request a referral authorization prior to receiving follow-up services from a non-affiliated provider. 7 Do yourself a health favor and get rewards* Wellness incentives The State of Wisconsin Group Health Insurance Program Well Wisconsin Program and Security Health Plan encourage you to visit your primary care provider, and to learn your cholesterol, blood pressure and blood glucose numbers. This is important for your lifelong health. There are two options for you to receive your Well Wisconsin Incentive. Option 1: Attend a Well Wisconsin worksite biometric screening event. Your results will be sent directly to Security Health Plan and may take 45 days to be received. Visit wellwisconsin.wi.gov for a calendar of screenings. Option 2: Visit your primary care provider for a preventive visit along with glucose and cholesterol screening. Your provider must fax us the Biometric Screening Results form which is included in your member materials. It is also available at www.securityhealth.org/state. After you have your results, complete your Security Health Plan Online health assessment. Each State of Wisconsin employee and spouse who is enrolled in your plan are eligible to receive the $150 incentive. Incentives will be mailed by the end of the month after the end of the quarter. For example, March 31 is the last day of the first quarter. You will be paid by the end of April. You must complete your annual health assessment and biometrics within the calendar year to be paid.
