Operating as - DOGLOGIC CANINE EDUCATION
RELEASE AND HOLD HARMLESS AGREEMENT
That for and in consideration of the services rendered by Lynne Stephens, Karen Mielke, partners and employees of PADS Inc llc, the
undersigned does hereby waive and release the aforementioned , for any loss, damage or injury to any dog, persons or property that may occur
from any cause as a result of taking part in this behavioral training activity. I recognize that there may be an element of risk in dog behavioral
training /handling. I understand that, as the owner of the Board / Train dog, I am responsible and legally liable for my dog’s actions for the life
of the dog, including the time period after Board / training. I understand that the success of any behavior training program is dependent upon
numerous factors including, but not limited to: the amount of time I spend working with my dog, how thorough I am in implementing the
training protocol, the previous history and experience of my dog, how the dog’s environment is managed and the genetics and structure of my
dog. I understand that the training my dog receives does not guarantee the dog’s behavior in the future and that dog behavior is affected by the
training that I implement, the dog’s previous training history, the dog’s temperament, the dog’s developmental stages and the dog’s
environment.
I understand that treat / reward based training will be used with my dog. This training method will be demonstrated to me at the end of the board
/ train period. I understand that I will be expected to continue my dog’s training in this manner on my return home and at all times in the future.
I hereby waive and release Lynne Stephens, Karen Mielke, partners and employees of PADS Inc llc from any injury or damage to my dog and I
expressly assume the risk of any such damage or injury during or following the Board / training period.
I also hereby waive and release Lynne Stephens, Karen Mielke, partners and employees of PADS Inc llc from any injury or damage resulting
from the action of my dog and I expressly assume the risk of any such damage or injury during or following the Board / training period. I hereby
agree to indemnify and hold harmless release Lynne Stephens, Karen Mielke, partners and employees of PADS Inc llc from any and all claims
by any member of my family or any other person accompanying me to PADS Inc llc as a result of the action of any dog, including my own. I
authorize Lynne Stephens and Karen Mielke to work with and care for my dog. I also authorize Lynne Stephens and Karen Mielke to assign a
PADS Inc llc partner or employee to work with and care for my dog under their direction.
I also authorize Lynne Stephens, Karen Mielke, any PADS Inc llc partner, employee or responsible person assigned by them to take my dog to
the veterinarian for any emergency. I will pay for all veterinary bills related to my dog, or any bills of another dog that my dog harms.
I certify that my dog does not jump over or dig under fences and has never jumped over or dug out of a fence and escaped the yard. I hereby
waive Lynne Stephens, Karen Mielke, partners and employees of PADS Inc llc from any injury or damage to my or any other dog as a result of
my dog escaping from the premises.
I hereby authorize the partners and employees of PADS Inc llc to transport my dog for a “field trip” if this is deemed necessary for training
purposes. I hereby waive and release PADS Inc llc, its partners and employees from any injury or damage resulting from such field trips.
I will pick up my dog on the agreed date. I realize that if I do not pick up my dog on the agreed collection day, I will be held liable for an
additional boarding fee of $45 per day for each additional day. If I do not pick up my dog within 48 hours of the mutually agreed date, I will
relinquish ownership of the dog. Should this occur, Lynne Stephens and Karen Mielke reserve the right to place the dog in another home or
relinquish it to a rescue society. An additional $500 rehoming fee will be charged for this service. I realize that I will also remain responsible
for all other training fees at the rate listed in this contract.
I understand that the tuition rates are as follows and agree to pay for the total amount by the first day of Board / training.
BOARDING ONLY (Inclusive of all walks)
Daily rate:
$30 per day for first dog in kennel
$15 per day for second and subsequent dogs in same kennel (up to a limit of 3 dogs per kennel)
Dogs sharing a kennel must currently live together with no history of aggression.
TRAINING RATES (charged in addition to the boarding rates above)
$20 per training session per dog per day – up to a maximum of three training sessions per day.
I agree to all the terms and requirements of PADS Inc llc’s Board / training programs as listed in the DogLogic Canine Education Terms and
Requirements document and I understand that the Board / training tuition fee does not include food, veterinary care or professional grooming
feesI have read and understand the above waiver.
