PUBLIC GUIDE TO UGANDA PASSPORT SERVICES
Uganda travel documents may only be held by citizens of Uganda.
They are processed and issued only by Government of Uganda
through the Directorate of Immigration Control (DCIC) and at
three Ugandan Missions abroad: Washington DC (United States
of America), London (United Kingdom) and Pretoria (Republic of
South Africa).
We have three Passport Control Offices in Kampala, Mbale
and Mbarara, The fourth Office, Gulu, will be operational at the
beginning of FY 2016/16. The main Passport Office is situated at
the Ministry of Internal Affairs Plot 75 Jinja Road (entrance is on
Old Portbell Rd. The other Passport Offices are located at the RDC
Building Mbarara, The Regional Administration Block, Republic
Street, Mbale and in Gulu (soon opening).
We also have Regional Immigration Offices which act as passport
Application Collection Centers, where you can take your passport
application for processing and then you collect a ready Passport.
These include: Jinja, Masaka, Fort Portal, Arua, Hoima, Masindi,
Gulu (soon to become a full Passport Control Office) and Lira.
REQUIREMENTS FOR A NEW PASSPORT APPLICATION. 1. Fully filled forms A and B, dully endorsed by Local Council
1,2,3, DISO and RDC;
2. Covering letter from Employer/School/Parents/ Guardian or
person well known to the applicant addressed to the Passport
Control Officer;
3. Photocopy and original identification documents e.g.
National ID, Voters Card, Birth Certificates, School Certificate,
Residential ID, Baptism Card or Employer’s Card;
4. Recommendation from a citizen of Uganda of good repute.
She/he shall provide you with a copy of his/her ID or Passport;
5. Proof of payment of prescribed fees (UGX 150,000 normal
applications, or UGX 300,000 for Express applications);
6. Any other information as may be required by the Passport
Control Officer.
REQUIREMENTS FOR RENEWAL OF UGANDA PASSPORT
1. Covering letter addressed to the Passport Control Officer;
2. Form A and B dully filled. Applicant may omit Local council
endorsements except where specifically required by the
Passport Control Officer;
3. The recommender on form A should be of good repute and
should provide a copy of his
ID or passport and copies
of the passport;
4. Copy of the old passport and any other relevant documents;
5. Proof of payment of prescribed fees (UGX 150,000 normal
applications, or UGX 300,000 for Express applications).
REQUIREMENTS FOR CHANGE OF NAMES OR OTHER
PARTICULARS
1. Written explanation for change of names addressed to the
Passport Control Officer;
2. Copy of a gazette or National ID
3. Dully filled Forms A and B, LCs endorsement may be omitted
except where specifically required by the Passport Control
Officer;
4. 3 colored passport size photos taken against a white
background;
5. Proof of payment of prescribed fees (UGX 150,000 normal
applications, or UGX 300,000 for Express applications).
PASSPORT APPLICATION FOR MINORS
1. Duly filled Form A and B including LC endorsement;
2. Written consent letter from parents addressed to the Passport
Control Officer;
3. If child is adopted, attach adoption order from High Court of
Uganda;
4. Copy of child’s birth certificate;
5. Copy of parent’s passport or Identification documents;
6. 3 colored passport size photos taken against a white
background;
7. Proof of payment of prescribed fees (UGX 150,000 normal
applications, or UGX 300,000 for Express applications).
PASSPORT APPLICATION REQUIREMENTS FOR STUDENTS
1. Duly filled Form A and B including LC endorsement from area
of residence or locality of study;
2. Covering letter from Institution of learning;
3. Copy of school Identification;

4. 3 coloured passport size photos taken against a white background;
5. Recommender on form A should be either Head, or lecturer or
administrator in institution of learning;
6. Proof of payment of prescribed fees (UGX 150,000 normal applications,
or UGX 300,000 for Express applications).
REQUIREMENTS FOR LOST PASSPORT
1. Dully completed form A and B. Local Council endorsements may be
omitted except where specifically required by the Passport Control
Officer;
2. Attach own and detailed written explanation indicating circumstances
under which loss of passport occurred;
3. Complete Questionnaire form;
4. Complete Affidavit in lieu of lost passport;
5. Police report confirming loss of passport;
6. 3 colored passport size photos taken against a white background;
7. Copy of Identification documents;
8. Attach copy of lost passport if available;
9. Proof of payment of a fine of UGX 100,000 and prescribed fees (UGX
150,000 normal applications, or UGX 300,000 for Express applications).

REQUIREMENTS FOR REPLACEMENT OF DAMAGED OR MUTILATED
PASSPORT
1. Written explanation under which passport was damaged / mutilated;
2. Dully filled Form A and B, LC endorsement may be omitted except
where specifically required by the Passport Controller;
3. 3 colored passport size photos taken against a white background;
4. Copy of Identification documents;
5. Damaged passport if available;
6. Proof of payment of a fine of UGX 100,000 and prescribed fees (UGX
150,000 normal applications, or UGX 300,000 for Express applications).
REQUIREMENTS FOR EAST AFRICAN PASSPORT
1. Duly endorsed forms A and B;
2. In case applicant holds a Ugandan Passport, attach a copy of the
passport showing the face and the file number;
3. Covering letter from Employer/School/Parents/ Guardian or person
well known to the applicant;
4. Copy and original of applicant’s identifications documents e.g. National
ID, Voters Card, Birth Certificates, School Certificate, Residential ID,
Baptism Card or Employer’s Card;
5. Recommendation from a citizen of good repute that shall provide you
with a copy of his/her ID or Passport;
REQUIREMENTS FOR CERTIFICATION OF UGANDA PASSPORT
1. 4 scanned colored copies of bio-data of passport to be certified;
2. Original passport;
3. Written request for certification addressed to the Passport Control
Officer;
4. 2 passport size photos;
5. Proof of payment of prescribed fees (UGX 20,000 per Copy).
