Attn: THE PRACTICE MANAGER A copy of a Letter of Wishes from your client is attached. Please place a copy of this letter with your client’s file and add the following information to their computer records. Your client has requested that their pet has an individual cremation with Nottingham Pet Crematorium. When the time arrives, please call Rita or John on 0115 981 6850 to arrange a collection. Following your call, we will collect the pet in a bespoke pet hearse. You will be asked to sign a document confirming collection and will be given a copy for your records; we recommend you keep this for at least 2 years. As soon as the pet arrives with us we will contact the client and make the final arrangements direct with them. They may request that we return their pet’s ashes to your practice or they may collect their pet’s ashes direct from us. Information about the Nottingham Pet Crematorium Nottingham Pet Crematorium is a specialist individual pet crematorium based in West Bridgford. We operate a bespoke funeral service where clients can bring their pet direct to the crematorium or have them collected on demand in our pet hearse. We offer attended or immediate cremation service enabling the owners to return home with their pet’s ashes later the same day. As members of the Association of Private Pet Cemeteries & Crematoria we are approved by DEFRA and fully comply with the animal by products directive.
