order personal health care products — at no additional cost to you!
When you enroll in a qualified health plan, you will receive Benefit
Credits you can use to order health care products from the Health
Products Benefit catalog. Orders are delivered right to your door —
at no cost to you.
Benefitt Credits are used to order
personal health care products
Benefit Credits are yours to spend each time you order items
in the Health Products Benefit catalog.
Here’s how it works…
• You will receive Benefit Credits when your
health plan coverage begins.
• Benefit Credits are then available in January,
April, July and October. Benefit Credits
expire on December 31 of each year.
• You will also get a catalog in the mail four
times a year. It has hundreds of health care
items, including vitamins; cough, cold and
allergy medicines, dental products, blood
pressure monitors; skin care items and more.
• You order products from the catalog using
your Benefit Credits. Best of all, your order
will be delivered right to your door — at
no cost to you.
No paperwork or receipts
The Health Products Benefit is easy to use. There is no paperwork
or receipts to turn in. We take care of everything. We even keep
track of how many Benefit Credits are in your account, so you
don’t have to.

How to place
an order
Two ways to order items from the catalog
1. Call a toll-free phone number. A Customer
Service Representative will take your order over the phone,
answer questions and tell you how many Benefit Credits you have.
2. Use the order form provided
in the catalog. Simply fill it out
and send it to us in the postage-paid
envelope.
Benefit Credits add up
When you enroll in a qualified heath plan,
you will receive Benefit Credits throughout
the year. Benefit Credits are yours to use;
there’s nothing you have to do to earn them.
Unused credits expire at the end of the year.
Choose from hundreds of
health care products
A catalog will arrive in the mail four times
each year. It contains hundreds of personal health care products you
can order whenever you like. Use your available Benefit Credits,
there’s no cost to you.
