Important Clinical Information Dear Students, As part of your course assessment you are required to attend Clinical Placements in various health facility settings. To participate, the Ministry of Health requires all students to satisfy several verification items. If you do not meet Ministry of Health Requirements, you will not be permitted to attend your clinical placement, putting you at risk of failing your clinical placement and subsequently your subject. To be ready for your assessments in time you must commence immediately on the following items. Please ensure that you familise yourself with the UNurse Guide (http://www.uts.edu.au/currentstudents/health/clinical-practice/clinical-placement-locations/clinical-verifications) that contains all the information required for clinical, there is an expectation that you have read this guide. Please also refer to UTS Online CLINICAL site for further information and clinical placement information. Take this letter to your doctor. VACCINATION RECORD CARD All vaccinations and immunisations must be recorded on a Vaccination Record Card with: vaccination name batch numbers dates of vaccination. Get your vaccination card at the Clinical Practice Unit (CPU) at Kuring Gai campus in Building 5, Level 3 or UTS Student Centre building 10, level 2, City campus. KEY DATES DEADLINE Before 31 Jan 2015 Before 31 Jan 2015 Before 31 Jan 2015 REQUIREMENT Apply for your National Criminal Record Check International students must apply for Australian Federal Police Check (AFP), these checks can be applied for online: http://www.afp.gov.au/en/what-we-do/policechecks/national-police-checks.aspx Local students need to only apply for NSW Police Force Check: https://npcoapr.police.nsw.gov.au/aspx/dataentry/Introduction.aspx Local students need to apply online with the above link, but the lodgement process will not begin until the student has attended a local police station to have their identification documents verified. Apply for your Working With Children check (WWCC) This application is free and can be done online (please select ‘volunteer’ option): http://www.kidsguardian.nsw.gov.au/Working-with-children/working-with-childrencheck Following the application, you will have to present the application number to the RMS and have your ID verified. The application will not be complete until you have done so. Find a location at: http://www.service.nsw.gov.au/service-centre *It is your responsibility to keep a record of this WWCC number and to take with you on the first day of placement. Visit a doctor or Vaccine Provider to receive your first immunisation doses Record doses on the vaccination card with the details listed above. The Ministry of Health policy requires the below vaccinations as a minimum to commence clinical placement – you cannot attend your first clinical placement without them: o dTpa: Adult single dose st o Hepatitis B: 1 dose (OR have the vaccination details transcribed in the Vaccination Record Card) st o MMR: 1 dose OR positive serology st o Varicella: 1 dose OR positive serology Visit a doctor to receive your second immunisation doses Record doses on the vaccination card with details listed above Ensure each entry is signed & stamped by a doctor. nd o Hepatitis B: 2 dose (if required) nd o MMR: 2 dose (if required) nd o Varicella: 2 dose (if required) *If required: proceed to TB screening and immunisation. You need to start this process if you are born or have lived more than 3 months (cumulative) in any of the listed countries shaded in dark blue from the list below: http://www.health.nsw.gov.au/infectious/tuberculosis/documents/countriesincidence.pdf **Do not commence TB screening prior to the vaccination process as it cannot be done in the 4 weeks prior to having live viral vaccines (such as MMR and Varicella). Students who require TB screening must attend a NSW Health chest clinic in their local health district of their residence. These clinics are often busy and it is recommended that an appointment be made for a time after you have completed your MMR & Varicella doses as per the above. Students can attend Follow up Bulk Compliance sessions at South Eastern Sydney Local health District facilities at the locations, dates and time listed on UTSOnline under the COMPLIANCE SESSIONS tab.
