Capability Avenue passionately believes in the possibilities
that lie in front of everyone. Our team is dedicated to making
those a reality. Our highly skilled and experienced team
members deliver supports and services for individuals and their
families living with a range of physical, intellectual, behavioural
and sensory disabilities.
Capability Avenue expands the experience and support of the Cerebral
Palsy Alliance to have a bigger impact for local communities. We have
had more than 70 years of experience, trail blazing in intervention and
therapies for people living with cerebral palsy and their families.
During that time we have had the opportunity to work with children
and adults living with a range of disabilities, not just cerebral palsy.
Our staff and therapists are enthusiastic about providing an outstanding
service, making sessions fun and engaging, and finding the right therapy
for each individual. We recognise the diversity in families and are
respectful of ethnic and cultural needs.
We have state of the art facilities where we deliver a range of supports,
physical activities, sports, information sessions, planning supports and
events. We believe we can all work together to achieve remarkable things.

access to the right service to meet their needs. The National
Disability Insurance Scheme (NDIS) opens the doors to greater
choice and control, which offers us the chance to work in
partnership to maximise your tailored individual plan.
Capability Avenue is registered under the NDIS. By choosing Capability
Avenue, you will have access to our highly skilled and dedicated staff
who offer a range of services that can be personalised just for you.
Capability Avenue offers you the full scope of services, in one
convenient location. We can work with you to offer a personalised
family-centred approach to; early intervention, individual therapy,
coordination of supports, counselling, in-home care, equipment,
life-skills programs, provided by highly trained experts focussed on you.

Early Childhood Intervention
Transition to School
At Capability Avenue, we are experts in providing early childhood
intervention services to children living with a broad range of disabilities,
including; developmental delay, intellectual disabilities, autism,
physical disabilities, multiple disabilities, brain or spinal injuries and
specific communication disabilities that require assistive technology.
Capability Avenue supports children and their families in attending
school, to make the start of school as smooth as possible. For the
family, this requires early planning and consultation with many people.
Early intervention teachers and therapists help preschool children
and their families to be prepared for the best start at school. We can
provide advice about modifying the school environment and support
your child’s individual needs.
Through our group and individual sessions, our early intervention
teachers and therapists are able to help children develop skills and
strategies necessary for successful learning and development.
We support children to participate as fully as possible in their local
preschool, day care program or school.
We provide early intervention services for children up to the age of six years.

Your client relationship manager will work with you to assess your
individual needs and a dedicated team of highly qualified therapists
will help you and your family to achieve your goals. This team may
comprise of:
Capability Avenue offers flexible support and education for families with
a child or teenager with a disability. Services we can provide include
counselling, parent education and assistance to coordinate your
supports.
Capability Avenue’s behaviour support services are available to children,
teenagers and adults living with a disability, as well as their families
and carers.
• A speech pathologist who will focus on communication which includes;
communication with technology, speech development, language and
literacy development and can also focus on swallowing, eating and
drinking.
We can explore ways to help your family to:
Through Capability Avenue you can access counselling sessions to
support you and your family. Through these sessions you may clarify
views, attitudes, feelings and thoughts in relation to life events and
relationships. Counselling can offer the opportunity for you to reflect
on your actions, thoughts and behaviours, to set personal goals and to
locate strategies to resolve these issues.
•
A physiotherapist will work with you to build and improve on
movement, physical development milestones and may offer advice on
exercises, activities and equipment that can assist.
• An occupational therapist can assist with aids and equipment for home,
school or work and work with you to develop a program to support your
developmental, sensory or independence needs and goals.
• Identify what is working well, what you would like to change, and assist
with developing strategies that will make a difference
• Manage stress, behavioural problems, relationship issues and living
with the effects of a disability
• Feel more confident, plan daily routines, access useful information and
practical support.
Behaviour support is usually needed when a person’s behaviour is
interfering with their daily life, their ability to form relationships, or their
capacity to cope with or manage social situations.
Our behaviour support team will work with you to understand why
the behaviour might be occurring, what triggers might be causing the
behaviour and how the person can be supported or managed differently
into the future.
We offer counselling for many reasons and common examples include
support for dealing with challenging experiences, such as receiving a
diagnosis of disability, or managing the stress of being a caregiver.
Our behaviour support team use evidence-based practices to assist you
and your support network to lead a happier and more meaningful life.
We focus on the needs of the whole family, with the aim of achieving and
maintaining the emotional well-being of all family members.
• A psychologist and/or social worker may help and support you or your
child to maintain positive emotional wellbeing / wellness and work with
to ensure your/your child’s learning and vocational needs are being met.

Assistive Technology
Lifestyle and Person Supports
Physical Health and Wellbeing
There is a wide range of equipment and devices, often referred to as
assistive technology, available to assist you in your day-to-day life and
Capability Avenue works with people with many different disabilities to
help them find the right solution for them. This technology may be as
simple as common household items such as cutlery to more complex
products, for example a speech-generating device for communication.
At Capability Avenue we are committed to supporting you to achieve
your goals. This could be through finding a job, enrolling in a course,
participating in social and recreational activities, catching up with friends
or getting out and about in the community. We provide 24 hours of
care every day. Whatever you need, whenever you need it, we are here
to assist.
Many people living with a disability require assistive technology to do
everyday activities and to promote their independence.
One of our disability support practitioners can assist you with your
personal care needs such as showering, getting dressed, having a meal,
getting up and going to bed, grooming and personal hygiene. This is
sometimes known as attendant care. If you are planning a trip or holiday,
one of our Capability Avenue team can talk to you about how we can
support you to make the most of your trip.
Maintaining your physical well being involves regular exercise, good
nutrition and making healthy lifestyle choices. Getting active is an
important part of incorporating wellbeing into your daily routine.
At Capability Avenue we have a team of exercise physiologists and
physiotherapists who can assist you through a variety of programs aimed
at optimising your physical wellbeing.
At Capability Avenue our dietician has extensive experience working
with people with disabilities and understands the impact of disability on
diet. Our dietician will work with you to assess your nutritional needs;
advise on nutrition and diet and implement and support you to manage
this change.
Capability Avenue can also provide you with support in your home
to help with cooking, cleaning, making the beds, pet care, grocery
shopping, paying bills and other jobs around your house.
Home-based care for children can be provided and help a parent or
carer have some time out for activities that may include attending an
event, shopping or a meal out.

Our Speech Pathologists
Our Psychologists
Our speech pathologists are experts in addressing challenges with
communication, literacy, eating, drinking & swallowing.
Your speech pathologist will work with you to assess your current
communication and explore ways to expand that communication. This
could include individual or group sessions focussing on:
• The development of language
• Literacy development
• Introduction of technology or augmentative communication tools
• Parent/carer communication partner training
• Speech clarity
• Communication with different communication partners and different
environments.
This may involve individual sessions to improve communication skills or
to support this communication.
Our speech pathologists can also provide assessment and
recommendations to support you in maximising your safety and
enjoyment during mealtimes.
Capability Avenue psychologists are industry leaders in supporting
people with their learning, emotional and behavioural needs. We work
with you to develop a plan tailored to your circumstances and
individual goals.
Working with you and your support network, our psychologists offer:
• Counselling
• Advice on addressing behaviours of concern
• Help to overcome difficulties, such as anxiety and depression
• Help and support to maintain emotional wellness
• Individual and group positive parenting programs
• Early identification and referral to address anxiety, depression and
relationship difficulties
• Consultation and advice on learning and vocational needs.
We offer psychology in group or individual sessions. We always work
towards the goals set by you.

Our Occupational Therapists
Our Physiotherapists
Our occupational therapists are experts in supporting people to improve
their independence and to address challenges with daily living activities.
Working with you and your support network, our occupational therapists
offer:
• Support with the development of fine motor (hand) skills
• Creative ideas to help make daily living easier for example, dressing or
independent eating
• Aids and equipment for home, school and work
• Support to help you participate in recreation and leisure activities
• Advice on major home modifications and minor home modifications
such as rails, entrances, pathways, ramps or steps.
We offer occupational therapy in group or individual sessions. We will
work towards the goals set by you.
Our physiotherapists work with children, teenagers and adults living with
a disability to work towards and achieve their personal goals. Our team
will support:
• The development of new motor skills
• Improve or maintain existing skills and abilities
• Support participation in sport and recreation
• Leisure activities.
Our physiotherapists assess how a person moves and identify factors
that limit the achievement of an individual’s goals. They may offer advice
on exercises, therapy and equipment when working with you as an
individual and your support network.

Our Social Workers
Our Exercise Physiologists
Our Disability Support Practitioners
Our social workers offer counselling, emotional and practical support
to individuals and their families. Particularly, when there are multiple
and complex challenges you are facing, social workers can assist you to
manage and locate appropriate resources in a timely way.
Working with you and your support network, our social workers offer:
• Counselling
• Education and information
• Support groups
• Referral to appropriate services and resources.
Social workers facilitate programs for individuals, siblings and carers/
parents to promote connections with like-minded people, resolution
of challenging issues and to promote wellbeing.
Our exercise physiologists work with teenagers and adults, their families
and carers to maintain and improve health and fitness for functional
tasks, as well as maintaining a healthy lifestyle. Working with you to
achieve your goals, our exercise physiologists can support you with:
• Goal directed strength and fitness programs
• Working with personal trainers
• Weight loss programs
• Healthy lifestyle education
• Task specific skill development and chronic disease management
• Participating in other sport, recreation and leisure activities.
Our disability support practitioners encourage, support and enable
people living with a disability with a range of everyday activities which
include but are not limited to:
• Personal support
• Mealtime preparation
• Domestic support and personal care.
We offer an individualised approach which includes organisational
support to attend appointments or recreational activities.
We are available at times which suits you, any day of the week & hours
that meet your needs.
